What Does My Blood Test Report Indicate?

He has extreme fatigue which does not seem to be improving. He had pneumonia a couple of months ago but his symptoms predate that.
On his blood test results the following
His results
WBC ... ..................3.89 ........... Normal range 4.00-11.00
neutrophils...........2.03................... 2.00-7.50
ALT...GPT.............54 IU/L 0 - 50
TSH .................. 45 miu/L 0.50 -4.20
Ferritin...............378 ng/ml 16-215
Despite going to the GP on many occasions , He is not feeling any better. Following eating every meal , his stomach bloats to the point that it hurts him. The doctor gave him Diflucan which helped a little but I feel that there must be an underlying problem. would really appreciate if you could suggest what it might be.
Thanks
Hello sir following are my recommendations-
Detailed Answer:
Hello sir,
I did review your query and few lab reports. Chronic fatigue especially accompanied by night sweats need attention.
Night sweats can be seen in many types of chronic infections, bone marrow disorders, chronic inflammatory conditions and so on.
His wbc count seems to be mildly low. with borderline low neutrophil count.
I would like to see your complete blood report including hemoglobin and platelet count.
Also your ferritin is high which indicates either chronic inflammation or iron overload.
In this case it is prudent to get and ESR and CRP done to see if there is chronic inflammation in some part of body or not.
Also, has he noticed any swelling in any part of body, or any abdominal pain?
Also does he have high grade fever spells and/or weight loss?
Please let me know the answers of these questions and I will be in a better position to suggest you future investigations and possible problems.
I will be waiting for the complete reports and answers to the questions I asked.
I hope this helps.
Wish you all the best.
Let me know if you have any more questions or concerns.
Sincerely,
Dr Parth Desai.

thank you for your reply.
Yes he has abdominal pain and swelling of the stomach especially after eating, even the smallest amount of food.
The sweating happens every night and also during the day when he feels a build up of heat and it always starts at the base of his head.
I will try and send the balance of his blood test results now.
looking forward to hearing from you
Hello, following are my recommendations-
Detailed Answer:
Hello sir,
I did review your reply answers and also the blood reports.
The reports essentially show normal hemoglobin and platelet counts which is a good sign.
However, mild leukopenia with high ferritin with normal CRP needs to be investigated especially for iron overload.
I would recommend the following-
1) An ultrasound of abdomen to look for any liver or spleen abnormalities(also since SGPT is mildly elevated and spleen mainly to look for increased spleen size which can cause stomach swelling with eating)
2) After Ultrasound of abdomen I would recommend to consult a hematologist and if needed get a bone marrow aspirate and biopsy done to look for iron overload.
Nightsweats are a part of constituitional symptoms and with other laboratory abnormalities are indicative of early bone marrow problems and the main suspicion would be hemochromatosis with increased serum ferritin and elevated SGPT. However, serum ferritin is also a chronic inflammatory marker and can be increased in many other conditions of inflammation. Generally in Hemochromatosis, serum ferritin is very high (in 1000s), however, it is better to do a bone marrow study and rule out the suspicion.
I hope this helps.
let me know the followup results.
Let me know if you have any more questions or concerns.
Wish you all the best.
