What Does My Blood Test Report Indicate?

Question: Date of test: 11th December
Results:
Iron 11.2 u/mol/L (10.6-28.3)
Transferrin saturation 15% (20-55)
T.I.B.C 76 umol/L (41-72umol/L)

Repeated on 23rd december
Iron 9.8 umol/L ( 10.6 - 28.3 umol/L)
Transferrin saturation 14%
T.I.B.C 69 (umol/L)

Doc can i just get your thoughts on my tests. First test Iron was within range. Trans saturation was low.
On the second test iron levels and transferrin saturation was low. The test was repeated after two weeks- what does this result suggest. Have I got iron problems? why would results be lower within 2 weeks?

Thanks for posting your query on HealthcareMagic.

The report on December 11 shows serum iron on lower side of normal range, low transferrin saturation and high total iron binding capacity (TIBC).
This picture suggests an iron deficient state.
The report on December 23 shows low iron, low transferrin saturation and TIBC within range, however TIBC is on higher side of the range.
Both test results suggest iron deficiency.
You need to take iron supplements to replenish the iron stores in body.
Besides these tests, serum ferritin may also be done.
As iron supplements begin, the iron tests should be repeated at an interval of one month.

The thing is I am a male and I am 2 and half stone overweight. I have had some abdominal pain but after a physical exam all has been cleared by the doctor.

I did have 3 tubes of blood removed on the 11th of December.. my problems started after treatment with Ciprofloxacin 500mg tabs and Diclofenac, which caused my system to mess up!

I am worried after your initial that my iron stores are low, can this be linked to internal bleeding? as my hematology report is okay.

What supplement do you recommend and foods can I try ? Also my ESR is 2. Which shows there is no inflammation, could this be a intestine problem?

Only RDW is high, all other parameters normal.

Detailed Answer:
Hello and welcome again,
Your hemogram shows all parameters are within normal range except RDW (red cell distribution width).
There is mild increase in RDW which shows that there is mild variation in sizes of red blood cells.
All other parameters- hemoglobin, red blood cell count, hematocrit, MCV, MCH and MCHC are within range.
Thus, the iron deficiency is not severe, it is just mild.
A ferritin level will be helpful.
Taking iron rich food is recommended, however, whether iron supplements will be required or not can be better decided by ferritin levels.

Drawing three tubes of blood will not affect your iron levels.

Low iron is caused due to decreased intake of iron rich foods, decreased absorption of iron or increased loss of iron from body.
Your hemoglobin is good enough, thus there is no evidence to suggest any internal bleeding.
Diclofenac is drug belonging to aspirin family.
When taken for long duration can lead to gastric bleed and this may become a cause of loss of blood from body and thus low iron levels.

The cause of low iron may be determined by clinical correlation.
As of now you can take up ferritin levels.
If ferritin is found to be low, iron supplements may be started after prescription and under supervision of your physician.

One final question as everything is fine, including haemoglobin etc - can i be sure there is no internal bleeding? as everything else is ok? My doctor has ruled this out.

From my blood test results am I also healthy?

Also I know the RDW value if repeated may even be normal, as you can imagine lab references vary.

Are there ways to reduce rdw such as exercise?

All other parameters mentioned are within normal biological range.
RDW is marginally raised, however, there is nothing to worry about it.

Exercise has no role in correcting RDW.
A little more iron in diet will get it in range.
