What Does My Blood Test Report Indicate?

Question: Hi Doctor,

During health checkup of my mother , aged 57 years old, following observations were done ( I am quoting only abnormal results). Rest were normal.

Ultrasound - mild spleen enlarged - 13 cm.

CBC WITH PERIPHERERAL SMEAR, ESR AND EDTA -

Haemoglobin 11.4 range - 12-15
MCH 25.8 range 27-32
MCHC 30.6 range 31.5- 34.5
RDW - 17 range 11.6 - 14

LYMPHOCYTES - 17.4 range 20-40

ESR ( WESTERGREEN) -22, range <20

Blood picture :
rbc - normocytic normochromic
wbc - count within normal range
platelet adequate

LFT :
ALKALINE PHOSPHATASE 148 range 40-129
GGT 6 range 8 -37


She is on diabetes medicine Jalra M and tyhronorm 50. Her sugar level is well within control.Her physical condition is good. no fever, loss of weight, or any abnormal symptoms.

I am worried with these findings. Is there something that needs to be done here. Please advise.
Somewhere on internet I read about enlarged spleen and leukemia and this has made me worried.

Thanks for posting your query on HealthcareMagic.

The blood parameters- hemoglobin, MCH, MCHC are low and RDW is high.
This picture suggests anemia and variation in sizes of RBCs.
The examination of the peripheral blood shows presence of normocytic normochromic red blood cells.
The anemia is currently mild but may turn severe.
You may get blood levels of iron and ferritin.
In case there is deficiency of iron, iron supplements may be taken to correct the deficiency.

Mild splenomegaly can occur due to viral or bacterial infections.
The ESR is also mildly elevated, this may be due to infection which is causing mild splenomegaly.
Splenomegaly is associated with leukemia and lymphoma , however, in that case it is associated with other constitutional symptoms.

Alkaline phosphatase is an enzyme with many iso-forms.
It can increase in liver disease, bone disease etc.
Osteoporosis is expected at this age and this can be the cause of mild elevation of alkaline phosphatase.
GGT is mildly reduced but this is not much consequence.

The glucose level is under control and this is absolutely fine.

History is not suggestive of leukemia.

Hello and welcome again,
Leukemia is associated with other constitutional symptoms like weakness, fatigue, decreased appetite, fever, recurrent infections and bleeding tendency.
Splenomegaly alone is not path-gnomonic of leukemia.
There are no such features mentioned in your history which raise suspicion of leukemia.

Sometimes, after working in kitchen she says about tiredness, fatigue. But she has no fever in recent times, no bleeding tendencies, no decrease in appetite , no weight loss and she does not shows frequent infections. Should I consider tiredness and fatigue as age induced effects and nothing more as her reports are fine.

Tiredness and fatigue are expected since there is anemia (low hemoglobin).
An iron profile consisting of serum iron, total iron binding capacity, % saturation and ferritin will resolve the matter.
In case there is iron deficiency, iron supplements acn be taken after prescription from your doctor.
