Question: Hello,

I am 42 year old male and my weight is 80kgs, height is 6 feet, did blood test today for cholesterol and my total cholesterol is 217 with non HDL 169, LDL 147. I want to know how bad these readings are and can these cause heart attack, very anxious.

Also I am an active person , go for regular runs and fairly muscular. I drink alcohol socially and not daily, also did my fast blood sugar test and it was 85.

Please let me know are my cholesterol levels very bad and do i need statins???

Brief Answer:
No diet and lifestyle changes to be followed initially for 3 months.

Detailed Answer:
Hi,
Thanks for asking your query on this forum.
I am Dr Rishu Saxena a cardiologist and i would be answering your query.
As per the query no your Cholestrol and LDL are just borderline elevated. You need not to start statins right now.
Initially follow diet modification plus exercise;for 3 months and most likely your levels will return back to normal again.
As you are already running regularly make sure that the speed of running should not be less then 5.1km/hr.(just running will not help).
Do strenuous exercises at least 30 minutes a day for minimum 6 days in a week.
Use less oil in food.Try to use olive oil if possible.
Eat lots of raw vegetable salad daily.
Take one cup of fruits daily they are rich in natural anti oxidants and will help you in dyslipidemia.

At present you need not to take any medicines.
Just avoid fats in your diet and follow what i suggested you above.
Things should return back to normal in 3 months.

In case you have any other query feel free to ask i would be glad to answer.

Regards.

Hello Doctor,

Thanks for your reply

Just wanted to tell you i jog on treadmill at speed of 9ks to 10Kms, is that ok?

Also do suryanamaskar yoga 2 times a week.

Also checked my bp it was 110/70 on electronic bp machines at home, the reading was an average reading taking bp on it 3 times and pulse rate of 54.

My fasting blood sugar is 85, based on all above with borderline high cholesterol levels, do you think i am strong candidate for an attack?

thanks in advance for reply.

Detailed Answer:
Hi again,
Going on your history you are not at all a strong candidate for coronary artery disease.Yet as i told you earlier on mail,TMT is highly sensitive to rule out lateral wall ischemia of heart.
All the three leads V4,V5 and V6 are representing the lateral wall and have significant ST-T changes.
Therefore you should go for stress thallim.

If it had been only inferior wall or septal then i would not have suggested you to go for thallium.
Regards.

Detailed Answer:
Hello dear,
Yes, as per your query.

Running on the treadmill at a speed pf 10km/hr is excellent but time duration should be at least 15 minutes.

For cardiac and lipid abnormality even running at 5.1km/hr for 30 minutes a day will do.
Blood pressure pulse and sugar is in excellent control.

As per the current history, you fall under mild cardiac risk category(practically not at cardiac risk).
The only risk factor you have is that you are borderline dyslipidemia.

Just follow what I've said earlier, lipids should be normal in 2 months.

Above the age of 25 years as per the standard protocol treadmill test (to look for ST-T changes particularly in lateral leads in ECG i.e. v5 and v6) and 2D-echo should be done every year to rule out any cardiac issue.

Feel free to ask further.

Regards
