What Does My Echo Test Report Indicate?
![default](/r/images/default.png)
My father, XXXX, was diagnosed with high BP and Mitral valve related Mitral stenosis condition 2 years back and has been taking BP & Cholesterol medication since then for which dosage has not changed since then and both parameters remain under control. He was started on Diuretic later on which is also continued. Most recently he was diagnosed with Gout condition due to higher Uric Acid causing frequent inflammation in his legs and hence taking its medication also. Because of Gout his Diuretic was changed from Lasilactone to Lasic which he is taking currently.
He has been having his ECG and Echo 2D tests done every 6 months and consulting to Cardiologist to monitor his Mitral Valve condition and has not been having any physical symptoms of discomfort till now. He goes for regular morning walks and avoids any strenuous exercises or physical activity. Based on his recent Echo report we want to take advice on his Mitral Valve condition and whether any further treatment is required at this stage.
please attach echo report
Detailed Answer:
Hello Mr.Trivedi,
I have gone through to your query. Thanks for using HCM.
As understand from your history that Blood pressure and cholesterol are fairly under control and his present medication are good enough.
As far as mitral stenosis is concerned there are 3 options
1.medical management which includes lasix and concor.which he is already taking.
2 Balloon valuloplasty I e.dilating narrowed valve with help of balloon.
3.surgical dilatation or replacement.
The decision of choosing one option depends on echo report.I need to know whether stenosis is mild, moderate or severe.
Also I need to know morphology of valve and probable leakage.
I request you to send me Echo report so that I can guide you properly.
You may continue medication for gout,
My best wishes
Dr.Rajesh Teli ,MD.

you need to consult interventional cardiologist
Detailed Answer:
Hello Mr.Trivedi,
I have gone through the reports of echo.As per the report he has severe narrowing of mitral valve. It is mentioned that he has severe calcium on tips of mitral leaflets.Usually presence of calcium on valve makes valve unsuitable for Balloon valvotomy. However I suggest you to consult an interventional cardiologist, who routinely performs balloon valvotomy, for echo evaluation. Many experts can perform valvotomy in presence of calcium on tips if the commissures of valve are free.So it is better if echo evaluation is done by such an expert. If valve is not suitable for valvotomy then he is candidate for surgical replacement in that case he will need pre operative coronary angiography which will be performed by interventional cardiologist.
So I request you to consult DM interventional cardiologist for decision making,
My best wishes

Can you advice on any precautions that can be taken before we decide on any interventional treatment or surgery and if decision can be taken after 2-3 months?
Also, if you can suggest an expert interventional cardiologist in XXXXXXX or XXXXXXX it would be helpful.
you may wait for 2 to 3 months
Detailed Answer:
Hello Mr.Trivedi,
First of all He must avoid exertion and take salt restricted diet .He must continue all medicines without any break. If he does not have symptoms like shortness of breath and swelling on feet you may wait for 2 to 3 months.Though the echo report is suggestive of significant lesion requiring quick action.
Dr XXXXXXX Dalvi at Glenmark hospital XXXXXXX or any cardiologist at XXXXXXX Hrudalaya Banglore should be consulted for the same,
My best wishes
Dr.Teli.
