Chest Xray is clear
No weight loss, temprature etc. - no health problem

TB PCR with the facility to test MDR confirms or rules out.

Read and understood the confusion between sarcoidosis and tuberculosis.
This is the biggest clinical dilemma.
My thoughts:
- If you are staying in XXXXXXX tuberculosis has to be given the more weight although sarcoidosis is not very uncommon.
- TB PCR or RNA study is most confirmatory of TB and the material should be sent for this test. Left axillary lymph node can be the easiest and even FNAC sample may be sufficient and easily available. This tells whether this is TB and also tell whether this is sensitive to the routine anti-tubercular medicines or is MDR.
Go for this tests and the diagnosis will be confirmed.
