
What Does My Kidney Ultrasound Report Indicate?

A couple of weeks back, after coming back home from Gym I noticed there was blood in my Urine (without any pain).
I went for a Ultrasound the next day and it was normal everywhere except it showed a grade I Fatty Liver with tiny concretions in bilateral renal pelvices.
However, later that day I felt a sudden pain in my lower right side of back between the ribs and thigh and has to be operated (Got a painkiller injection).
I took medications for the pain (Voveren and anti-acid syrup Citralka with Cystone).
A week later I felt a lot of pain while urinating so I checked in to see my doctor who checked me for UTI with urine routine and culture test, which were perfectly fine. He also recommended me Dynapres Capsule (0.4) and a new Ultrasound.
Thw new ultrasound of the kidney and bladder showed :
"There is a calculus of size 7.9 x 4.6mm in the distal right ureter close to right UVJ. There is associated grade one Hydronephrosis in Right Kidney with mild Right Side Hydroureter".
There were of course also tiny concretions in bilateral Renal Pelvices as stated earlier.
Right Kidney is 105x43x17
Left Kidney is 99x48x18
Prostate is 13cc
Bladder is well distended and normal.
My blood test are also perfectly fine and haven't had any previous diseases.
What should I do now?
Should I opt in for the medical treatment i.e taking pills, drinking fluids to eradicate the stone since it is very close to bladder (distal ureter) though is quite big?
Or Should I go in for surgical treatments and If yes, then which one would be wiser?
Other than the stone Do I have anything to worry about, hydronephrosis or fatty liver?
Dieting Recommendations?
Currently I have only mild pain after urinating and a mild backache. I feel tired and clumsy and have trouble masturbating.

I have gone carefully through the details as well as the reports. You have probably having the kidney/ureteric stones for sometime which have caused back pressure leading to hydronephrosis. This has subsequently manifested with bleeding with urine as well as the pain that you have mentioned.
Unless the stones are removed, your problems are not going to resolve. Based on the size that you have mentioned, a process called lithotripsy can be undertaken (in order to avoid surgery) wherein the stones are broken down into smaller particles with help of laser/ultrasonic vibratory waves and hence does not require surgery. Based on the location, there is more likelihood of it passing out smoothly with urine after the stone gets disintegrated. Talk to your doctor if that facility is available and whether it is feasible. If not feasible, surgery is the only option as I would neither suggest you to wait, nor would I insist on trying any medication. However, drinking plenty of water and fluids is essential.
You need not worry about the fatty liver for the time being as it is probably an unrelated and incidental finding. But if you are drinking alcohol, you need to cut down on it.
The hydronephrosis is due to the stone obstructing the path of the urine and so, you need not worry about and treat the hydronephrosis separately.
About diet, I would suggest low protein intake for the time being. Drink plenty of fluids and water. No other specific suggestions. You can continue taking alkalinising agents like Citralka.

As you suggested I went to ask my doctor about lithotripsy, he suggested me a X-Ray but the stone was not visible on the reports. Therefore lithotripsy was not possible.
Since the stone is at last stage (In UV Junction) he suggested me to look out for one more week and then do another Ultrasound.
Is there any chance of passing the stone through drinking fluids?
How will I know that a stone has passed?
Will it break into pieces or just come out as it is?

The stones can have various compositions but if it is not calcium based, it is not visible in x ray. But this does not mean lithotripsy cannot be done. It is just that the outcome of lithotripsy cannot be assessed properly and hence the proposal not to go for it.
Since the size of the stone is small, there is significant possibility of it being passed out intact (not in broken form) if you drink plenty of water/fluids. Often it goes undetected but you might observe the pain going away or on a follow up ultrasound. If the pain is not severe, you may wait for a week or more.


As my stones were not seen in X-Ray is it a fact that they are not calcium based, I guess the calcium ones are the hardest to break with diet or medicines. So that's a good thing?
Also after few days of heavy intake of fluids the pain has gone away and I am able to sleep at night and urinate astoundingly. Though, there are shivers around ureter once a day, so I'm not sure.
Can I delay my surgery to the end of month?

Thanks for writing back. The stones do not break on their own. So, it is neither good or bad. It is good to hear that the pain has come down with increase in fluid intake. If the pain is not bothering you, you can probably do without surgery and with high fluid intake. Chances are that it will come out on its own. But if the pain recurrs, you need to visit your doctor without delay.
