What Does My Lab Test Report Indicate?

default
Posted on Thu, 28 Jan 2016
Question: What does this ultrasound report mean?

Background: I am currently 14w3d pregnant. I had a detailed ultrasound after a necessary surgery on tuesday. This is the report:

Crown-rump length is 8.25 mm corresponding to 14 weeks 1 day. Fetal heart is 137 bpm. There is posterior placenta which is low lying w/ marginal previa.

There is nonspecific thickening of the nuchal fold measuring up to 5.1 mm. Cannot exclude cystic hygroma. Limited evaluation demonstrates large cystic structures demonstrated with anterior cranial vault. Etiology is indeterminate and could reflect underdevelopment/abnormal development of anterior brain parenchyma versus abnormal compression of brain parenchyma versus hydrocephalus. Recommendation is for further evaluation by a high risk OB.

My ob spoke w/ us but i could barely comprehend what he told me. I am desperate for info right now.

I took the harmony test several weeks ago and it game back at virtually zero risk for the things it tests for.

No abnormal heart rates....no weird hcg levels...in fact, the day before this scan I had a thorough scan at my regular OB and everything came back perfect. My OB mentioned they have higher quality ultrasounds at the hospital.

Brief Answer:
Need more information.

Detailed Answer:
Hello dear,
Thanks for trusting hcm about your health care.
Congratulations on your pregnancy!
I have gone through your history and understand your concerns.
In your report two things are important one is increased nuchal fold thickness,so cystic hygroma is the possibility and another possibility is down syndrome.
Cystic hygroma will resolve with pregnancy advancement but to rule out down syndrome triple marker test are important.
Have you done with those markers?
Other is low lying placenta, which can be shifted to upper segment with pregnancy advancement.
Are you having any vaginal bleeding or so?
Can you please upload your ultrasound report?
Hope to hear from you soon.
Good luck.

I have typed out what the report says verbatim. What about the cystic structures in the brain parenchyma?

I did a test that is 99% accurate for detecting down symdrome and a few other chromosomal abnormalities. This came back that I have virtually zero risk. I took this test as a precautionary measure weeks ago before receiving this ultrasound report.

Brief Answer:
It could be due to arachnoid and choroid plexus cyst.

Detailed Answer:
Hello dear,
Sorry for the delay in my response.
It could be due to choroid plexus or arachnoid cyst.
As these are not associated with any congenital anomalies in your case, maybe disappear with time and no intervention is required.
But you have to repeat your ultrasound after 2 weeks.
Hope found useful.
Good luck.

So choroid plexus or arachonoid cysts can appear in the anterior brain parenchyma?

I keep finding information saying that after 13 weeks 6 days the normal value for nuchal fold thickness is 6 mm. Therefore, could the 5.1mm in my ultrasound be normal?
doctor
Answered by Dr. Richa Agarwal (2 hours later)
Brief Answer:
It is more than 5 mm.

Detailed Answer:
Hello dear,
In my opinion if nuchal fold thickness is more than 5-6 mm,it is considered abnormal.
So in your case it is considered a borderline case but as your markers are negative, no need to worry.
But for more confirmation amniocentasis and second level ultrasound is more diagnostic.
Hope found useful.
Good luck.

Brief Answer:
You have done triple marker test for down syndrome.

Detailed Answer:
Hello dear,
As we discussed before I think you had already done your triple marker test.
If not which hormonal test you have done before?
Could you please tell me the report or upload the results?
Hope to hear from you soon.
Good luck.
