Question: Dear Mam / Sir,

Please see my attached report for second consultation for my cardiac related assessment.

Is everything perfect ??

Your symptoms do not suggest Coronary Artery Disease (CAD). However, it is better to have a Treadmill Exercise Electrocardiogram (TMT). If it is normal, nothing to do further

Your Fasting glucose is in upper limit… Have HbA1C and GTT (Glucose Tolerance Test) done to exclude diabetes…

Your LDL cholesterol is high…
There are subdivisions of cholesterol – LDL, HDL and so on.
HDL is good cholesterol – higher, the better.
LDL is bad cholesterol – lower, the better.
In the absence of any other risk factor 100 is ideal for LDL-C. In the presence of other diseases (especially Diabetes and/or established heart disease), 70 should be the aim.

     Keep your weight to normal / even a little less
     Quit smoking, if you are a smoker
     Do regular EXERCISE – say walking or jogging - 30 minutes a day / at least 5 days in a week. Isometric exercise (like bull worker) is bad.
     Healthy FOOD is a must. Avoid saturated fats – coconut oil, palm oil, butter, ghee, full cream milk...Avoid junk foods in particular – French fries have palm oil / Pizza has cheese / XXXXXXX sweets are made in ghee.../ Change to Poly-un-saturated oil – like sun flower oil. / Include Mono-un-saturated oil – Olive oil.
Repeat the test in 2 to 3 months.
     Cholesterol can still be high, in spite of diet and exercise – you need drugs; because it is also manufactured in the liver – and that is determined by heredity. Statin is the first choice. Some may start it straight away – even with normal levels, even without problems (Primary Prophylaxis)

Dear Sir,

Thanks for the reply.

I respect your recommendation but wish to know following answer point wise.

1.) Do I have the borderline sugar problem ??

2.) Do I have the border line cholesterol. ??

I just don't want to take medicine for long time so wish to reduce all these within limits by doing exercise only.

Also as all reports are within reference range as it seems as normal. Just to understand once.

1- Yes, you MIGHT High Fasting Glucose
Further testing is necessary to rule in or rule out – when the level is at or above 100

2 - Bad Cholesterol is not borderline – definitely HIGH / Your level is 125 - Normal less than 100 / Even 70, in the presence of other diseases / 130 was old standard and is not universally accepted nowadays
You need Life Style Changes, as already described
You MAY need drugs – IF the level is till high after 2 months…     
The need for drugs, if any, depends on re-testing – after diet and exercise

Dear Sir,

Thanks.

Lastly please suggest if any medicine to take for L.D.L Cholesterol from now ( If yes please suggest the dosage / after breakfast / before breakfast etc.).

Also to share that in my family no one has sugar / diabetes / cholesterol.

Secondly I take care that sugar level shall come down by changing my lifestyle.
What is URINE GLUCOSE ABSENT sir ( I understood that I have no sugar/diabetes).

( CAN I TAKE SWEET DISHES ETC. ) OR I SHALL STOP EATING NOW.

I am very thankful to you for this recommendation because early checkups and action can lead a very good life.

Hi

As said already, adopt Life style changes and repeat the test after 2 months.
Start medicine if the level is still high… but at the same time, continue the diet and exercise
The most common medicine is the statin class – there are several of them, the most widely used one is Atorvastatin / and the most common dos is 20 mg at night (after supper) – adjust the dose from 10 to 80 mg, depending on the progress

The problem might be with second degree relatives like aunt, uncle and so on..
And diet is contributing to the individual

Sugar in urine appears after a certain threshold only / the timing of food and specimen also affect
As it is, there is no definite Diabetes – but only an indication to test further and exclude

Avoid sweets – apart from sugar, they have plenty of saturated fats, which will add to bad cholesterol

You will have a long life indeed
