Brief Answer:
Need assessment of blood supply.

Detailed Answer:
Hi,

Thanks for your question.
Noted your concern.

Looking at pictures there is gangrenous changes in foot.
Gangrene occurs due to lack of blood supply.

Most of the time there may be need for surgical removal of gangrenous part called as amputation.

In such a case there is a need for assessment of vascular status which can be done by Arterial Doppler (one form of sonography) or CT peripheral angiography or peripheral angiography.

Treatment decision depends upon detailed physical examination, vascular assessment, available facilities.

Along with this Intravenous antibiotics, to improve blood flow use of vasodilaters, blood thinning agents may be suggested.
For better results there is a need for good blood sugar control (for which insulin treatment may be suggested), blood pressure control.

Better to consult foot surgeon and diabetologist.

Hope this helps you.

Dr. Abhay Mali.
