Detailed Answer:
Hi XXXXXXX
Welcome.
I just went through your reports & noticed that you have severely deranged lipid profile, an increased hs CRP & mildly deranged liver function.
Let us discuss one by one.
1) Seeing the lipid profile, it looks like it is hyperlipoprotenemia, which is a abnormality of lipid metabolism. Very high triglycerides (>500) is especially concerning & such patients are at high risk of pancreatitis (inflammation of pancreas).
So decreasing it soon will be our goal. Rest of parameters are deranged but medicine is same.
To start with, you should first follow some strict (i really mean strict) dietary restrictions. These are:
Stop drinking alcohol immediately, if you are drinking, decease juice & cold drink intake, decrease fat intake to minimum level. Remember things that give instant energy (purified carbohydrates) increase triglyceride level.
Daily exercise is a must now, for at least 30minutes per day.
For such patients, we start Tab Atorvastatin 20mg once a day after dinner. So you should also start that.
Remember again high triglycerides are serious & can cause life threatening condition pancreatitis.
2) hs CRP - It is a indicator of risk of cardiac events. And your's is high which means you are at high risk of getting a cardiac event.
For such patients, we advice for regular health monitoring in form of stress echo every year. So please schedule for a stress echo.
3) Deranged liver function in form of increased SGOT/PT & GGT is not that serious and could be because of fatty liver. I'll advice you for a Ultrasound of abdomen for same to rule out any stone.
Again treatment here is decrease fat intake. You can start Liv 52 twice daily for it.
As the medicine for treating abnormal lipid profile is excreted from liver, you have to recheck LFT after 1 month.
So start the above recommendations from today only. I am not making you panic, but you really need attention at this point. Their are specific medications to decrease high triglycerides, which many doctors do prescribe straight away, but you do not need them as per the current international guidelines for treatment of lipid disorder.
Repeat the lipid profile again after a month. Also remember, lipid profile is best measured after 18 hr fasting.
Rest of the tests are fine. You don't have diabetes & thyroid function is also normal.
So, good luck.
Wishing you to be fine & healthy soon.
Thanks
Take care

Thank you Dr. XXXXXXX for your expert advice. After your consultation I really felt worried and will strictly follow your advice in my daily routine.
Yesterday, i also showed my reports to Gastroenterologist, He adviced me all what you have adviced (along with Upper abdominal Ultrasound and LFT after 6 weeks).
However, he prescribed me the Tab FENOLIP 145 MG TABLET (Salt:Fenofibrate 145 mg) once a day. Request you to please advice if is this medicine is sufficient to go ahead or shall i go ahead with your prescribed medicine (Atorvastatin 20mg).
Thank you very much for your great help and expert advice. Looking forward for your answer.

Hi Mr XXXXXXX
Welcome back.
I am happy that you are willing to follow the advice.
You should go for Ultrasound abdomen as i told you to see for liver or probably stone. Repeat LFT after 1 month, that i also explained to you.
As i told you also many doctor go directly to fenofibrate(specific medication for high triglycerides), but current guidelines of XXXXXXX society of cardiology do not say so & recommend taking statin initially & later adding fenofibrate if triglycerides still remain high(and we in XXXXXXX follow this only).
So ill recommend you going for Tab Atorvastatin 20mg once a day. Repeat lipid profile after 2 months.
Thanks
Take care

Brief Answer:
At night
Detailed Answer:
Hi Mr XXXXXXX
Atorvastatin or any other statin should be taken at night before you sleep as its absorption is maximum at night. Dose is once daily.
So Tab Atorvastatin 20mg once daily at night.
Remember this drug is eliminated from liver & you should repeat the LFT after 1 month.
I hope i answered your query.
If you have any more question anytime, I'll be happy to help.
Thanks
