What Does My Neck Ultrasound Scan Test Report Indicate?

Question: I had a neck ultrasound today and the report shows:
1. Well defined hypoechoic nodular lesions are seen in both sides of neck at the level of angle of mandible.lesions of right side measures 8*12.7mm and6.8*10.8 mm.the lesions of the left measures 9.2*17mm.Evidence of necrosis is noted in the left sided lesion. What does it signifies and how serious it is?
2. Bilateral level II Cervical lymphadenopathy. What does it signifies and how serious is it?

Does it signifies TB or some infection?U can also check my earlier question for more details.

Hi Sir,

I understand your query and concern.

Your Ultrasound report is suggestive of probable Salivary gland infection (parotid), thyroid or any soft tissue tumor.Need to rule out malignancy.

An immediate Clinical examination with CT scan neck is needed for accurate diagnosis.

I can suggest you further if you provide me the following details:

1.Duration of swelling?
2.Associated with pain?
3.Any recent increase in size of swelling?
4.A cough, fever,change in voice? if present duration?

Please upload complete ultrasound images and reports.

Kindly find below the complete ultrasonography of neck:-

1.THYROID: Both lobes of the thyroid and isthmus are normal in size and shape.No focal solid or cystic lesion is noted.No Calcification is noted.

2.BILATERAL PAROTID:Are normal in echogenicity. No focal lesion is noted.No evidence of any ductal dilatation seen.No calcification or ductal calculus seen.The adjacent fat planes are maintained.

3.BILATERAL SUB MANDIBULAR SALIVARY GLANDS: Bilateral submandibular salivary glands are normal in size,shape and echogenicity.No calcification or ductal calculus noted.

IMPRESSION: Bilateral level II Cervical lymphadenopathy.

Regarding your points, kindly find below the clarification:-
1.No idea .
2.No harsh pain but there was a feeling of pain in the month of XXXXXXX 2015th and have to remove the chain around my neck and presently there is no pain.
3.Cannot determine the increase in swelling.
4.No cough, fever or change in Voice.BUT I AM GETTING ULCERS IN MY LIPS(INNER) FOR THE LAST 4 MONTHS.ALSO THERE IS A SINGLE AND PARTICULAR FORMATION OF MUCOCELE AT A SAME PLACE .IT GETS WELL AND AGAIN FORMS.
5.ULTRASOUND IMAGES CANNOT BE LOADED.

I understand your concern.

I have gone through your query in detail.

Malignancy of salivary glands can be comfortably ruled out based on the scan reports.

The possibility of TB lymphadenitis(Tuberculosis of lymph nodes-primary infection) is high.

This is also known as Scrofula.

Many of the symptoms can be vague and non-specific in most individuals.

This can be primary infection of the lymph node( in most cases) or secondary to chest TB in few cases.

Diagnostic confirmation is mandatory for further therapeutic management.

For this, I advise you to get FNAC of the involved lymph node along with Chest X-ray.

Further management is based on test results.

Ok , I Will do as the tests prescribed by you and is it 100 % curable?
Another question is there for you:-
I had a protected sexual encounter wit no oral sex with a call-girl on April 12th and had a HIV I and II Test which found to be non-reactive by western blot method on 06.07.15.Its well past 3 months and want to know how accurate it is?
Do I have to take a more HIV test?

Yes, scrofula is 100% curable.

Yes, western blot is pretty accurate.

You do not need further testing.

For more accuracy, you can go for PCR-RT for HIV 1,2.This test is best known to detect viral RNA.This is an optional test in your case.
