Question: my name is XXXX age 32 years, vegetarian and non-alcoholic but some time i eat tobacco daily.
please review my report and suggest me

ULTRASOUND OF ABDOMEN & PEĿVIS
LIVER Is normal in size (12.9 cms), shape, position & echo pattern: No focal lesion. Intrahepatic biliary radicles are not dilated. Portal vein is normal and shows hepatopetal flow. CBD is normal
GALL BLADDER
ls partially distended & shows a small echogenic focus of size 3.6 mm along the posterior wall of body portion with no posterior acoustic shadowing POLYP. Wall thickness is normal. No evidence of calculus chalecystitis.
SPLEEN is normal size (9.8 cms), shape & homogenous echopattern.
PANCREAS is normal in size, shape, position and echotexture. No pancreatic duct dilatation. No focal lesion / Calcification.
KIDNEYS Both kidneys normal in size (measures 9.0x 4.1 cms on right and 9.7x 4.9 cms on left side) shape, Position contours. They show normal cortical thickness, parenchymal thickness and cortico-medullary differentiation. No evidence of mass. No hydronephrosis or hydroureter

Right kidney shows a 3.6 mm calculus in the mid pole calyx.
Left kidney shows a 3,4 mm calculus in the mid pole calyx.
URINARY BLADDER is physiologically distended. Echofree lumen. No calculi / wall thickening
PROSTATE is normal in size and echotexture

No evidence of free fluid in peritoneal cavity. Significant sized lymphadenopathy.
Impression:
•     Small gall bladder polyp as described above.
•     Bilateral renal calyceal calculi without hydronephrosis.

I have read through your query in detail.
Please find my observations below.

1. Your liver is completely normal on ultrasound.

2. The gall bladder shows a 3.6 mm polyp. A polyp less than 6 mm is not a concern. However you can get another ultrasound done in 6 months to know if the polyp has increased in size. A polyp is a finger like growth and only those larger than 10 mm require to be treated by surgery.

3. There is one stone in each kidney at the interpolar region. The stones are about 3.5 mm in size and are not causing any obstruction to the passage of urine.

Tobacco chewing might lead to cancer and is best reduced slowly and then stopped completely.

You might choose to follow instructions below to avoid any more stones in your kidneys and to cause the stone to disappear.

DOs for kidney stones
(i) Drinking as much as 1.5 to 2.5 liters a day may help flush out your urinary system. This will also allow small stones to pass through.
(ii) Visit washrooms regularly and empty your bladder when you feel like doing urination.
(iii) Please take fruits like apple, papaya, watermelon, pineapple, sweetlime, bananas and orange, vegetables like, white gourd, ladyfinger, pumpkin, pulses and legumes.
(v) If you have sharp pain then you should consult your doctor and take pain reliever medications under strict supervision.

DONTs for kidney stones
(i) Please avoid holding urine for long intervals.
(ii) Please reduce foods containing oxalates including vegetables like spinach, tomato, cauliflower, brinjals, nuts like cashew, pista and almonds.
(iii) Decrease extra salt in foods.
(iv) Avoid spicy pickles.

Dear Dr, XXXXXXX ,
Thanks for the reply and giving the good suggestion. regarding kidney stone my doubt is clear .
But what about (The gall bladder shows a 3.6 mm polyp) what treatment should i follow now at home or any other testing is required for the same. what diet should i follow.
And one more thing i forgot to tell you that i have acidity and gas problem. for this i am taking homoeopathy medicine.

Thanks for writing back with an update.

Regarding the gall bladder polyp, if it is less than 6 mm then nothing is to be done. For polyp 6 mm to 9 mm you have to observe by doing another ultrasound scan after 6 months and for polyp more than 10 mm surgery is best.

As it is 3.5 mm the size is small. You can get another scan done in 6 months to know if it is still the same or growing in size.

No treatment is needed for this polyp at 3.5 mm.

Acidity is a common problem and has many solutions. If you are getting relief by taking homeopathy then please continue taking it. Have your meals on time and eat snacks frequently. Avoid spicy and oily food. Drink lots of water an=d fruit juices in summer.
