What Does My Semen Analysis Report Indicate?
Question: Hi Doctor,
My age is 30 & we are planning for baby. I have done semen analysis last week. No. of days of Abstinence is 3 days (contact done on Sat & semen test done on Tue). My report is as follows:
APPEARANCE: Normal
Liquefaction time: <30 min.
Consistency: 1
Volume: 2ml
ph: 7.5
WBC: NIL
Agglutination: NIL
Report comments states: 1-2 Sperms/HPF. severe oligoasthenoteratozoospermia
As per my doctor, as its severe oligoasthenoteratozoospermia there is no sperm count in the report & very minimum sperms were available. I had done the same test 8 months before as well, that time my details are as follows:
Sperm Count: 07mill/ml
Motility(%): 52%
Progressive Motile: 42%
Viability: 85%
Morphology: Normal-16%, Tail defect- 28%, Head Defect-42%, Neck defect-14%
Comments on report: Oligozoospermia (4M/ml Progressive motile sperms)
AFter this report doctor advised me to take 2 medicines i.e. Progenix & Lycostar (twice a day both). I have taken both for 8 months & had test again last week (which says severe oligoasthenoteratozoospermia). Now my doctor asked me for again repeating the Semen analysis test by maintaining the same 2-4 days gap.
How critical is my situation? Is there any way I can improve my sperm count? What should be my next step?
My details are as below:
Age:30
Height: 5.6 ft.
Weight: 110kgs.
Please advise & guide me.
Thanks & Regards,
AXXXXX

Brief Answer:
Oligoasthenospermia
Detailed Answer:
Hi
I read your query and I understand your concerns.
Following is my reply
1. Your sperm count is ranging between 0 to 10 million which is oligospermia.
2. The motility and morphology also seem to be low.
3. In such cases it is difficult to improve semen parameters by drug or any other way.
4. For pregnancy you may have to undergo ICSI procedure which is best option in men with low semen parameters like yours.
5. Meanwhile stop smoking and alcohol if you have habit.
6. Start weight loss regimen as semen parameters do improve in men with such severe oligoasthenospermia.

Hi
Your repeat sperm count is again lower. Hence ICSI is recommended.
Following is detailed reply for your questions
1. Success in ICSI depends on various aspects like your wife's egg quality, your sperm quality on that day and also other factors. It's around 30 to 50 percent.
2. The procedure involves multiple injections to your wife. But these are generally harmless and not much possibilities of side effects.
3. There are many hospitals in XXXXXXX which do this procedure.
4. The cost is between 1 to 2 lakhs.
5. Long gap of not having sex is not related to low sperm count.
6. It is difficult to improve such low count. I suggest you see a urologist in XXXXXXX and get eevaluatedand examined if there is room for improvement. They can examine and tell you.
