What Does My Semen Analysis Test Report Indicate?

Question: Hello doctor
Hope you are well...
My wife has had 2 missed abortions in the past one year (1st no heartbeat, 2nd time, heartbeat at 4 weeks but no heartbeat at 6 weeks). We conceived immediately both times, though my wife has a history of irregular periods..usually 30-45 days cycle with no specific trend.
Her age is 27 and we've been married for almost 6 years.
My wife has always had hormonal problems. Her LH was 8 & FSH was 5.86; IGG was 69 and IGM was 4.9.
Also, her Insulin was 27.8 and RBS was 90 .
My semen analysis also showed 40% abnormal and 40% active sperms 4 months ago. This has improved a bit after I took miraqule (Coenzyme Q10) 100mg for 2 months. Now it is 60% active and 30% abnormal sperms.
The gynae also did follicular monitoring for a week, where she found everything to be ok, except that there was some fluid before, during and after the periods (she called it Fluid in POD). She also said that my wife had 5 cysts which was not a bad sign at all.
She says, since, we have not found the exact root cause of 2 missed abortions till now, she can not allow us to conceive again to avoid a 3rd accident. She recommends going for a laparoscopy to rule out/fix multiple issues like TB, blockage etc. due to which the supply to the foetus may be falling short or any other issue thats not visible in USG or other tests. She wants also to have Thrombophilia profile & Premenstrual EB with/post laparoscopy.
Are we going in the right direction?
Thanks

Brief Answer:
Yes,you are going in the right direction.
Detailed Answer:
Hello dear,
Thanks for trusting hcm about your health care.
I can understand your concerns.
I have gone through your wife history and investigation reports and in my opinion your doctor's opinion is right, you should first find out the cause( if possible) than try again in future to avoid further misscarriage.
Usually first trimester abortion may be due to chromosomal defect and we are not able to find out the cause.
She should start taking folic acid tablet which is very helpful.
Hope it is useful, feel free to discuss more information.
Good luck.
Regards
