Brief Answer:
Very low count
Detailed Answer:
Hi,
Good to know you both are doctors.
Following are my observations:
1. Your husband's semen count is very very low which is difficult to improve.
2. Even sperm motility is very low.
3. In such situation ICSI ( Intracytoplasmic Sperm Injection) needs to be done. But even for that there should be few morphologically normal sperms.
4. I don't see any normal morphology. There is 100 percent head defects.
5. His sperms are bad and not advisable to be used for fertility with present scenario. Donor sperm is a better option.
6. If you wish to evaluate him further, please get his FSH, Testestesteone , prolactin and thyroid levels tested.
7. Get scrotal Doppler ultrasound scan for checking testicular size and Varicocele.
8. Please repeat his semen analysis is a different standard laboratory again to confirm if this report is true.
Let me know if you need any more help.
Regards,
Dr. Mahesh Koregol.

Brief Answer:
Repeat semen analysis
Detailed Answer:
Hi,
Proxeed or any other antioxidant is unlikely to help such a bad count.
Repeat semen analysis from different laboratory and see if it's so low.
Also get a scrotal ultrasound Doppler scan .

How about directly extracting sperm from testis for icsi? Is that possible?
What do u think is the most likely reason for such a bad count? Could it be reversible?

Brief Answer:
Yes Possible
Detailed Answer:
Hi,
You are right. TESA can be tried. But the quality of sperms are unlikely to be better.
It is difficult to state a reason without seeing doppler reports and blood test. Let me know the reports so that I can analyze and let you know.

Ok thanks, will do that next week in another centre.
He doesnt have any clinical sign or symptoms to suggest that he has varicocele, can patient having varicocele without any clinical feature? We are getting usg doppler done though.
He actually has family history of infertility. His two uncles (mother side) did IVF 10years back, one of them having 3daughters now, another uncle failed to have any kid because his wife experience ?eclamsia and had miscarriage at week 20. My husband's brother also trying to have baby for 2years but he has not gone for any checkup yet. All the women in the family ( his aunty and his sister) are fertile and raising own kids now.
Assuming my husband has some x-link kind of inherited infertility, can i be assured that there is still good possibility of doing ivf/icsi looking at his uncle's example?
Sorry for many questions

Brief Answer:
Better to use donor semen
Detailed Answer:
Hi,
Looking at this history, I am not sure results of ICSI. It is difficult to go for ICSI as the sperm count, morphology is not adequate for ICSI.
If you go for ICSI, please opt for PGS ( preimplantation genetic screening ) to rule out the chromosomal abnormalities in embryo and transfer only the healthy embryo. Discuss about this with your treating doctor.
Chromosomal abnormality is a possibility looking at the history.
