I can understand your concern.
I have gone through the results of your semen analysis.
1. The sperm count is with in the normal range (15-200 millions/mL)
2. Rapid progressive motility is NIL (Normal range: 32 - 72%)
3. Sluggish linear progressive motility 60% (should be more than 25%)
The extent of progressive sperm motility is related to pregnancy rates.
The combined rapid & sluggish progressive motility should be more than 50%
4. Total normal forms are 29% (Normal should be more than 60%)
So, abnormal forms are more than normal.
The report has to be clinically correlated (should be evaluated by the treating doctor).

In order to increase the motility & the normal forms of the sperms, the following measures are useful:
1. Avoid alcohol, smoking or stress
2. Take healthy balanced diet rich in protein & vegetable, low in fat, fruits, & minerals. Protein rich diet includes meat, fish, poultry (if you are a non-vegetarian), otherwise pulses like lentils, whole grains along with fruits and vegetables are useful. Dry fruits like walnuts, pistachio, melon seeds are also effective.
3. Avoid hot baths, hot tubs, or using lap top on your lap for long time
4. Wear loose comfortable cotton undergarments.
5. Keep your weight in check and do regular exercise
