Brief Answer:
Suggestive of infection with normal sperm count & motility

Detailed Answer:
Dear XXXXX, Hi
Good evening.
Thanks for posting your query HCM.
I have gone through the history given in your query and the uploaded results of semen analysis in detail.

The report suggests infection with normal sperm count & motility.
Other factors like sperm morphology are within normal limits.
Infection is indicated by the presence of pus cells in semen.
Normally there shouldn't be any pus cells in semen.
Infection may be due to UTI (urinary tract infection), prostatitis, etc.
You need a course of appropriate antibiotics for infection after the culture & sensitivity test of the semen.
You need few more investigations like routine hemogram, RBS, Ultrasound of pelvis should be done.Trans rectal sonography of prostate can be done if needed.
As far as your marital life is concerned, you are just married in February 2015, and the fact that she is having painful menses (dysmenorrhea), wait for a couple of months. It is a treatable condition and not responsible for infertility.

To increase the chances of conception (pregnancy), have regular sex on days 12 to 16 of her menstrual cycles becausse the ovulation (release of egg from the ovary) occurs on day 14 or 15 of menstrual cycle.
You should do exercise and take nutritious food.
You should avoid tight underpants.
Avoid cigarettes and alcohol if you take these things.

Hope the suggestions given above could be of great help.

Best regards.

Brief Answer:
An appropriate course of antibiotic after the culture & sensitivity test

Detailed Answer:

Dear XXXX, Hi
Good evening.
Thanks for your reply. Noted the contents.
Here are the answers for your query:

1. The best treatment would be to get the culture and sensitivity test done on semen and based on the results an appropriate antibiotic to be prescribed by your treating doctor. Or else till you get the result of the culture and sensitivity test you may take Tab Ciprofloxacin 500 mg twice daily for 5 days after consulting with your doctor.

Drink plenty of water, cranberry juice, wear loose cotton underwear to promote air circulation and reduce the chances of bacterial growth.
cranberry juice
2. Cause of infertility (wife not getting pregnant) does not appear to be the pus cells in the semen. Your semen analysis is normal.
3. This is due to urinary tract infection
4. You are advised to consult a urologist for better evaluation & treatment
5.There are many good Urology hospitals in XXXXXXX like Apollo, Life Care Medical Centre & urologists like Dr. Raji Goel or Dr. XXXXXXX Khaitan.
6. Anti-inflammtory drugs like Mefenemic acid or Naproxen containing medications are effective to control the painful menses. Some times combined oral contraceptive pills are also needed.

However, I would advise you to taken an opinion of a gynecologist for your wife's problem, as the cause of painful menses should be detected (ultrasound scan) and treated accordingly.

Hope the suggestions given above could be of great help to your and your wife.
Best wishes.
