Thanks for your query ,based on the facts and report of semen analysis that you have posted it appears that he has what is called as Oligoasthanozoospermia meaning there by that his his sperm count and percentage of active grade one motile sperms is lower than what is accepted range for normal effective fertilization to effect in to pregnancy .
The presence of 12-15 puss cells strongly suggests infection either in Seminal Vesicle or Prostate and needs to treated with appropriate antibiotics .
Get his semen culture done and take the appropriate antibiotics for 4-6 weeks .
Truly speaking there are no medications that can increase sperm count or sperm motility .However following measure will help you to increase sperm count and sperm motility to some extent .
1) Practice regular exercise for 45 minutes followed by meditation for 1/2 an hour in the morning.
2) Take high protein diet rich in vegetables and fruits and Vitamin A,C,D,E.and Zinc
3)Take anti oxidants like Almonds 5-6 everyday
4) Avoid alcohol and smoking..
Repeat semen analysis after 6 weeks of treatment if the parameters of semen remain on lower side Consult qualified Infertility specialist for guidance and help as regards exploring possibility of conception with the help of newly assisted reproductive technique like IVF or ICSI,

Also while giving sample his hands were not clean as he was driving bike and some dirt or something was in his hands so that might be the reason otherwise anything you can suggest to improve.

He can take combination of Sulphamethoxazole and Trimethoprim (Bactrin D.S.) twice daily for 4-6 weeks and repeat semen analysis after 6 weeks of treatment .

The report of semen analysis done at some other laboratory is contradictory to previous one .
I would suggest you to repeat it again after three days of abstinence at a renowned Infertility Laboratory where semen analysis is done by very sophisticated techniques by a trained experts.
The presence of 4-5 pus cells suggest infection and needs to take antibiotic suggested earlier .

I will briefly explain role of Varicocele played in Infertility .
Normally all the veins in lower extremities and testicles are provided with non returning valves which prevent column of blood to fall down in up right position .
During Varicocele or Varicose veins in legs (as you see in Rickshaw Pullers ) these non returning valves become in compitant resulting into blood to accumulate in these veins causing dilatation of tributaries of these veins what is called as Varicocele .The production of Sperms is controlled by a delicate balance between body temperature and atmospheric temperature .This balance is disturbed with Varicocele resulting into low sperm count .However there is no firm evidence to suggest this theory and so the results of the surgery for Varicocele are not guaranteed..It helps only in 30 % of cases and one has to take chance and be mentally prepared to accept the failure.
As regards the fertility boosters .I personally think it to be a pharmaceutical industry taking undue advantage of psychology of young couples and human mind who are desperate to have a child of their own.
