Question: Male 69 healthy no meds. Dr. Said I have bph slowing flow but the big problem is ultrasound showed an enlarged median lobe pressing into the bladder acting like a valve that completely stopped urine flow on one occasion. Considering green light laser or turp. Which would be the best to deal with that enlarged median lobe ? Also which would have less risk regarding erectile dysfunction?

Brief Answer:
Read Detail answer below.

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that you have obstructive voiding symptoms due to median lobe enlargement of Prostate.

TURP is the nomenclature of the same endoscopic procedure .TURP is an endoscopic procedure where enlarged prostate is resected by using either conventional electric current or Lase energy..The aims and objective of the procedure are same being to resect the prostate.

If the prostate is grossly enlarged and patient is economically sound we in XXXXXXX do it by using laser energy.
However in an isolated enlarged median lobe we normally do it with conventional electric current where charring of the prostate and surrounding tissue and post operative infection is comparatively less as compared to laser energy which burns the prostate tissue with current of very high voltage.

As regards your query about Erectile Dysfunction after surgery I would say that with advancement in instruments and expertise there is no risk of getting Erectile Dysfunction after surgery. However almost every patient develops what is called as retrograde ejaculation irrespective of method of prostate surgery.

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.
