What Does My Ultrasound Scan Test Report Indicate?

Question: Hi, I have got irregular periods from 2012, done with ultrasound in 2013. reports says that
uterus: retroverted. normal in size, shape & echogenecity.
uterine size: 6.4 * 3.2 * 3.3 cms
endometrial echoes are normal
endometrial thickness is 7.5 mm
right ovary(RO) : bulky in size.small hemorrhagic cyst noted measuring 1.7 * 1.8 * 1.4 cms.polycystic morphalogy noted.
Left ovary (LO) : normal in size.polycystic morphology noted
RO: 3.8 * 3.0 * 2.0 cms, volume 12.4 ml
LO: 3.0*2.4*1.7 cms, volume 6.6 ml..
no free fluid
no adenexal lesion seen
impression: polycystic morphology in both d ovaries, small hemorhagic cyst in RO..
I m trying to conceive.. I m on metformin from many days. at present 500 mg 3 times a day.. Also from 4 cycles taking clomid starting with 50mg,next 100 mg, 150 mg n this time with 200 mg. Still notable to ovulate. Is this really a problem with d above all details to conceive. I m really worried.. I am overweight also..I m 29+. started getting my periods regularly now.. but not ovulating.. plz suggest me something.. Thanks alot in advance sir/madam..

Thanks for the query to hcm,
This report is classical for polycystic ovarian syndrome where there is hyperandrogegism, polycystic ovaries and often obesity. Anovulation is a hallmark for PCOD. At present as you have already undergone ovulation induction by clomid and have been unsuccessful, i would suggest you to get the serum testosterone levels first.
If the levels are high then the best would be to control them first and then try for pregnancy as with obesity and without treatment for PCOD , the chances for pregnancy failure are high.
1. you may have to be placed on low dose oc pills for a period of 6 months for normalising the hormone levels.
2. decrease weight with exercise as this shall help decrease insulin resistance which then shall promote ovulation.
so kindly review with your testosterone reports.

Hello, thanks for ur reply.. How many lbs/kgs i need to reduce in order conceive? now i m 5 feet and 154 lbs.. any other alternate ways to get conceive..I am taking daily 1500 mg metformin+multivitamin tablets.. I was on clomid on 22nd to 26th of july month. tested with clearblue easy ovulation kit from 2 days but got the result negative. I was expecting the ovulation since my doctor prescibed 200 mg clomid... plz reply.. thank u..

Hello,
You are definitely overweight here with 70kgs at 5 feet tall. You need to reduce your weight by at least 10 kgs and i understand it cant be done in a short time, hence best would be to have patience and go through regularised treatment protocol of being on low dose oral contraceptive pills for 6 months and continue metformin and multivitamins. Weight reduction is the key to conception here in your case. Insulin resistance has to be brought down but even if with 1500 mg metformin daily that you are not responding to ovulation by clomid then i think you should stop ovulation induction and take to treatment first. Else you may have negative results and chances to conceive may be lesser.
This time with 200mg clomid if ovulation kit i negative till day 18 of your cycle then a follicular study should be done to check for the size of follicles which shall give a definite idea about the size and possible ovulation hence.

ok thanks alot for ur quick reply. Ok i llsurely try to reduce my weight. but these days i m getting periods regular i feel..my period dates from april are as follows.april 1st, may 15th, XXXXXXX 20th and july 19th.. was on clomid from 4 to 8 days (200 mg). After coming to US i had become 164 lbs.. now reduced 10 lbs with diet(GM diet). Tested for ovulationsince from just 2 days.. Ll test further also.. upto how many days i can test for ovulation? wen ll be my ovulation usually?

Hello,
Ovulation kit cane started to be used from day 10 of your cycle and in cases of irregular menses should be continued for 10 days till day 20 XXXXXXX or till positive result is obtained. This confirms LH SURGE but doesn't confirm ovulation at all.
Ovulation can be confirmed only with ultrasound follicular monitoring.

If the LH surge is high that means ovulation is about to start right? Here doctor doesnot do ovulation if we asked to do also. They say it takes time. Wat about my period dates? Is that seems to be regular or not sir.. Doctor asked to wait for some time and keep on metformin. I have married 3 yrs back..

The above report which i sent is scanned in may 2013.. Is there any chances of reducing cysts by this time? I m on metformin since last october 2013.. before that i have not taken any treatment..Thank u sir.

Sorry for the delayed reply,
Yes once LH SURGE occurs which gives a positive result on ovulation kit indicates that in next 36 hrs ovulation should occur. The period dates are fairly delayed and indicates a possibility of PCOD. If your scan was for 2013 , then you need to get a fresh ultrasound done to check if the cysts in ovaries are still present.
As you have used metformin for long now, a serum insulin level should also be done to check for present insulin sensitivity status.
Weight would have to be reduced. Avoid oily and spicy fast foods, increase your water intake to 12-15 glasses a day and practice morning and evening brisk walking for 45 min each with increased stress on salads and legumes.

They should have doe the scan to learn the present status and hormonal baseline.
Weight reduction and pills are to be taken yes but now that you were on it since 2013, a repeat hormonal profile was mandatory.
