Question: for Dr Rovena Murati
I am sending you my test reports which i have received . i will show these reports to doctor tomorrow because she was not availabale today. Kindly check and tell me .one value is out of limit . Kindly tell me is it diebeties?and whats u think all of my problems are due to this reason?

I have seen carefully the reports you have provided.

You have done an urinalysis ,which resulted completely normal.

The other test is done to evaluate the sugar in your blood.
The fasting blood sugar is 107 mg/dl and it is in the upper limit of normal range.Normal range is 70-110 mg/dl.

The other test is Glycated Haemoglobin (HbA1c) and this test is a common blood test used to diagnose diabetes.
The level of HbA1c in your blood reflects your average blood sugar level for the past two to three months.
The normal value for HbA1c in a person that doesn't have diabetes is between 4.5 -6 %.
A level of HbA1c over 6.5 % in two separate tests indicates diabetes but usually this test in not used to diagnose diabetes but only to monitor blood sugar in diabetics.
The recommended test for diagnosing diabetes is fasting blood sugar over 126 mg/dl in at least two separated tests.

In this measure your level of HbA1c was 6.9 % ,which is considered high.

For now this tests means that in the past three months the level of sugar in your blood has been high.Eating two much sugar and being on constant stress may have caused these high levels of sugar in blood.
The fact that your fasting blood sugar is not too high is a good thing.

Your doctor may just recommend you a strict diet and regular exercise to keep the blood sugar in normal range.
You may need to test again after 2-3 months of following diet and lifestyle modifications.

You should start a strict diet and physical activity. You doctor will recommend the adequate diet for you.

But now one value is above tha pre diabetes means 6.9%. So kindly tell me what was the reason behind the test. means why doctor need diabetic test on my symptoms?
you can imagine when i was stress what could be the level of stress like fear of hiv. does such type of extreme fear is enough to cause diabetes in a person ?
my father was diabetic patient but after 50 yrs of age. is it due to genetics? i cant understand why i have because other then last 3 month i was always away from sweets . and my weight is also very less. only remaining reason is stress.
kindly tell me is it possible that my stomach problems are related to diabetes?
because i think that the muscle twitching are due to diabetes and after these twitches i got more stress.

First of all ,the diagnosis of diabetes is not established .You have a high level of blood sugar in last three months but to diagnose diabetes is necessary that further test confirm fasting blood sugar over 126 mg/dl.

It is possible that stressful situations can precipitate diabetes in a patient predisposed to have diabetes.Genetic predisposition in a known risk factor for having diabetes.This means that having a parent with type 2 diabetes puts you on greater risk of having diabetes in your life than general population.

Diabetes is a systemic disease and can affect all body organs.High levels of sugar in blood can affect gastrointestinal movements and causing symptoms .
So diabetes can cause stomach symptoms and muscle twitching but these symptoms occur mostly after several years of uncontrolled diabetes.
Your blood sugar levels are over the normal range but not high enough to be diagnosed as diabetes and to cause such symptoms.
This doesn't exclude the possibility that the cause of your symptoms if this high level of blood sugar.

The best way to control your blood sugar is too eat healthy and avoid all foods high in carbohydrates and exercise regularly.

ok dr thanks for reply and detailed answer. i will tell you the details after discussion with doctor.
i have some random thoughts kindly reply because i know i am again and again sending same type of questions.
Is it possible that i have diabetes or pre diabetes from last year?
and as you told me that after many years diabetes can cause problems.
but doctor directly thought about diabetes and my sugar was high. during this stress time period as you know i told you near about 4 month before . that i was feeling a pain in upper stomach near chest , and you told me that i should do some liver function test . is there any relation with these test?
i have only two symptoms stomach upset means soft stool one or two times a week and 2nd is muscle twitching.
3rd thing that i have noticed is at the start of stomach upset i was feeling less hungry . but now from last 2 month i am feeling more appetite and hunger . is that also a symptom of diebeties?

Yes ,it is possible that you might have high levels of blood sugar for some time now.It is difficult to say it for sure since you have not been tested before for blood sugar but the result of your tests now don't indicate very high level of blood sugar that would explain the symptoms.
Usually complications from diabetes happen after several years of uncontrolled diabetes (this means several years with high levels blood sugar like fasting blood sugar over 130 mg/dl ,HbA1c over 7 % ).

Some months before when you had the pain in the upper right abdominal pain ,I suggested to do liver function tests just to exclude any problem with liver considering the fact that liver conditions can cause such pain.The fact that your symptoms improved with diet ,Omeprazole and probiotics was indicative that most likely the cause of the pain was stomach.
Liver tests are not related to the tests you have done recently.

Increased appetite is a symptoms of diabetes associated with weight loss ,frequent urination and excessive thirst.
High levels of sugar in blood might cause gastrointestinal disturbances and possible muscle twitches (although this is not typical in the beginning of diabetes).
In this case with the management to low the levels of sugar in blood ,theses symptoms will improve.
But it is possible that the gastrointestinal symptoms are not related to diabetes and are symptoms of IBS.

I would appreciate if you keep me informed how will go the consult with your doctor.

My doctor told me 6.9 number is high, it should be under 6 , and i have to take diet with many changes , like less sweet and calories diet , not even sweet fruits like bnanna & XXXXXXX daily 15 min walk etc and after one month i have to do these test again if this level still not down then medicine will be given,
Dr can you please tell me that i can eat chicken and egg? And rice also? Because these are the most common things availabale here.
My also told me that there is no sugar in urine so its not serious thing, but 6.9 % shows that there is something little problem in pancreas ,i am tealy confuse now whats this, and where i am standing now, if past 3 month glucose level was high then hows possible i am not having diabetes , and one thing i told you that i was having a pain under my right feet is that also related to this.?
Now from yesterdest i have a little slight pain in my right side under jaw or neck area, these things again gives me horrible thoughts

For now we can't tell if you have diabetes because the level of sugar in your blood is high but not high enough to be diabetes.
This means that in this stage it can be prediabetes and it can be controlled will proper diet and exercises.

The pancreas is an organ that produces a hormone called insulin.
This hormone is very important for the body because it makes possible that body cells use blood sugar to produce energy.
Insulin helps keeps your blood sugar level from getting too high (hyperglycemia) .So the direct action of insulin is to induce the cells to use blood sugar ,and this lowers the level of sugar in blood.
When the pancreas doesn't produce enough insulin ,the levels of sugar in blood increase causing diabetes.
When the level of sugar in blood stays high for long time , it can damage blood vessels and nerves causing complications.

I agree with the recommendations of your doctor.The level of blood sugar in your case is not too high and can be lowered in normal values with a strict diet and exercises.
The diet should consist of 40% to 60% of calories from carbohydrates, 20% from protein and 30% or less from fat. It should be low in cholesterol, low in salt and low in added sugar.
Foods high in carbohydrates are fruits, vegetables, beans, dairy foods and starchy foods like bread,potatoes ,rice .Try to eat fresh fruits, vegetables,whole grains, legumes, and low-fat milk is encouraged.
-You can take proteins from poultry and fish .You can eat chicken but without skin and fat.
Avoid saturated and trans fats ,which are found in solid fats like cheese, red meats, butter, margarine, and shortening.Instead use olive oil, fish,.
-Avoid fatty processed foods, fried foods.
Egg yolks are high in cholesterol so you can eat them only occasionally.

You can eat mostly all types of food but the key is how much you should eat.
You probably should consult a dietitian to help you with your diet by determining what foods you should eat and how much for every meal and sneak calculating the amount of carbohydrates ,proteins and fat for every food.
She can work with you in a certain diet plan taking in consideration the available foods there.
You should exercise at least 30 min a day most days of the week.

It is unlikely that the pain under the feet and in your neck area is related with the high level of blood sugar.

Try to relax and just follow strictly the diet and lifestyle changes.I am sure this will be enough to lower the blood sugar in normal values.

Yes you are right should manage by diet changes, you know the main reason of my stress , now ever 3 days i have a soft stool that gives me more stress that i am having some serious problem, and plus these muscle twitching and last week over thinking of my neck gland, i will try to manage diet but i am failed to control health anxiety type these things, many of these are actual problems like i was having pimples, stomach up set , muscle twitching and before i was having muscle pains.
and more thing how can i check which type of diabetes is this? or there is some special test for it?
does any viral infection can cause diabetes ?
and should i purchase a glucose meter?

I can understand your concern and that some symptoms like soft stools flare up every time you are stressed.
Since you have done stool examination and was every thing clear ,it means that nothing serious is causing the episodes of soft stool.The most possible cause is IBS that is triggered from stress and anxiety.

In you case ,the diagnosis of diabetes is not established because your blood sugar is not high enough to be diagnosed as diabetes.
If the level of blood sugar increases and Diabetes is confirmed ,it would be Type 2 diabetes.

It is true that several studies have concluded that Diabetes can be triggered from a viral infection.This is true for Type 1 Diabetes.
During certain viral infections ,out immune system produces autoantibodies to fight the infection.It can happen that the structure of specific viruses is similar to some part of pancreas cell that produce insulin .During this viral infection, the autoantibodies will attack and destroy these pancreas cells leading to diabetes.

In this case the pancreas doesn't produce insulin and the person needs to be treated with insulin injections.

For now you don't need a glucometer.You should follow a strict diet with exercises for 1 month and then get tested again for the level of sugar in your blood.

As you told me that most chances are my muscle twitchings are not due to
High level of sugar. Then my muscle twitchings reason is anxiety? Is it possible that anxiet can casuse muscle twitching? nurological possibility is there or not? Because yesterrday after walk and little exercise if felt more twitchings, on thighs arm , i felt sometimes a little buzz type feeling like vibration. Is it due to anxiety?
If i have last 3 month high level of sugar , then is it possible i have high sugar level from many months?

To answer your question,yes it is possible that you might have high blood sugar from several months.We can't know if for sure since you haven't been testes before.
Even if you have had this level of blood sugar for many months ,this wouldn't cause you to have diabetes complications because for this is necessary a high level of glucose in blood for several years to damage blood vessels and nerves.

It is possible that the muscle twitches are due to anxiety ,most if other cause have been excluded like vitamin deficiency, calcium imbalances, thyroid problems and neurological causes.
In your case it is really unlikely that your muscle twitches are due to high blood sugar.
If the muscle twitches are persistent and don't seem to improve ,you may need evaluation like thyroid function tests , neurological examination.

I would suggest to give it some time and see if with diet and exercises the muscle twitches will resolve.
If high blood sugar is the cause of the muscle twitches (although this is unusual but not impossible) they will improve gradually as your blood sugar lowers in normal range.

If the muscle twitches don't seem to improve and persist even when your blood sugar is near normal range ,then get the investigations done.

If everything is normal ,then anxiety and stress might be the cause.
