What Does Noisy Breath And Pain While Swallowing Saliva Indicate?

Question: Hi,

My name is XXXX, 38 years.
I dont know what my problem is. Quite often it happens to me, While swallowing my own saliva, my breathe becomes noisy and cant talk and do anything for a while.
After 1 or 2 minutes sound goes away and comes to normal condition

what is the problem ? what is the treatment ?

Hi,


I had gone through your question and understand your concerns that you are having difficulty in breath during swallowing. I would like to ask few more details.

Is there any burning sensation in chest ?
Is there any regurgitation of partially digested food from stomach ?


Kindly get back to me with the details.


Based on your clinical details, it appears to be aspiration secondary to disturbance of swallowing mechanism due to acid reflux.


During normal swallowing mechanism, vocal cards ( voice box) has to close. whenever liquids or own saliva touches and enters vocal cards, a reflex mechanism occurs, where person can not speak, cough out the saliva.


I suggest you to follow these measures to overcome your swallowing issues.


I suggest you to to consult ent surgeon and give detailed history (acid reflux) get laryngoscopy ( voice box abnormalities and acid laryngitis secondary to aspiration). depending on the examination reports further treatment is required accordingly.

Mean time, start ppi inhibitor with pro kinetic combination (pan- d) (pantoprazole + domperifone) twice daily before food ( over the counter). Reduction of weight (if you are obese), Do not speak during swallowing, give pause between talking lengthy sentences.

There is NO burning sensation in chest.
There is NO regurgitation of partially digested food from stomach.

Iam scared during that time whether my breath becomes normal or not.
The day when this happens Iam scared to sleep in night because sometimes it happened while sleeping also.
Is this a life threatening ?
So what should be my eating habits & how my life style should be ?
ENT doctor found acid inflammation in throat and prescribed PANSIO DSR capsules once in morning before food for 15 days. shall i stop this ? and continue what you have prescribed?

1. Take small 6 meals instead of large 3. Cut down coffee and bicarbonate breverages. Keep 3 hours interval between dinner and sleep, Keep head end elevation (support with extra pillow, to prevent regurgitation). avoid spicy, junk foods.

2. Continue the medication (pansio -dsr) as per your treating doctor.

3. Intractable aspiration can have recurrent chest infections (it can be life threatening in voice box abnormality secondary to tumors of larynx).
