Question: I'm a 70 year old female with diastolic heart problems.. I had an episode with out of my body, far away stare and slowness of speech. Never lost consciousness and was aware of my surroundings. Episode lasted for about five or minutes.

With your described symptoms of some changes in your speech and perception, whille being with consciousness, mandates exclusion of organic issues. This is an episode that can be considered as a transient ischaemic attack (TIA) in your brain, which is characterized by a temporary bout of neurological symptoms, which result from reduced blood supply to the brain, such as a carotid artery blockages, blood clots etc.

Therefore, I would suggest these tests, so we can further evaluate this situation:
- echocardiogram - to assess the motility and structural integrity of all chambers in the heart; also valvular function; to evaluate if there is a reason that this heart can generate and throw thrombi (blood clots) to the periphery;
- echoDoppler of the carotid arteries - to assess for blockages in these arteries, which could produce a low blood flow to the brain arteries;
- computed tomography (CT) of the head - to see if there is any blood clot in the brain bloodstream and to assess the brain areas form their composure, to further evaluate if this a primary condition, and not secondary to reduced blood flow.
- complete biochemical panel - to see if there are any abnormalities in glucose, liver function tests, electrolytes blood levels
- complete blood count - to see if there is a reduced hemoglobin level
- a consult with a neurologist, to further evaluate the symptoms and test results.
