Brief Answer:
Mostly due to infection of Prostate.(Prostatitis)

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that your friend has acute infection of prostate (Prostatitis) .

I shall be happy if you can post me few more details to help me in making comments on his problem.
1) Does he have urinary symptoms like frequency, burning urination, urgency etc?
2) Does he have fever with chills?
3) Did he have unprotected sex preceding these symptoms?

His symptoms are mostly due to Prostatitis.

Ask him to consult qualified Urologist for clinical and digital rectal examination and get following basic tests under his guidance.
1) Routine Urine and Urine Culture.
2) Semen examination and semen culture
3) Ultrasound Scanning of abdomen and pelvis .
4) Swab test and culture of the urethral discharge obtained after prostate massage .

Start taking broad spectrum antibiotics like Augmentin along with urinary antiseptic like Nitrofurantoin twice daily and anti inflammatory drug like Diclofenac twice daily Later on switch on to appropriate antibiotic as per culture report .

Ge the prescription from your family physician.

Ensure he drinks more water .This will help to keep urine dilute .

Hope I have answered your query ,please feel free to ask if you have more questions I shall be happy to help you
Thanks and Regards.
