What Does Tendinosis In An MRI Report Indicate?
Question: Hello Dr. Ditah. I don't understand why my shoulder MRI report says "there is tendinosis" in the supraspinatus tendon but the doctor and physiatrist keep referring to ""tendonitis" when they aren't the same thing. And I don't understand why the report doesn't specify HOW MUCH tendinosis there is, and whether it makes any difference, since the discomfort despite the cortisone injection must relate to contracted tightened tendon rather than inflammation. What do you think about this? Thanks.
Brief Answer:
Tendinosis = Chronic tendonitis!
Detailed Answer:
Hi and thanks for following up with me.
Tendinitis is an acute condition. After repeated bouts of tendinitis or extended acute tendinitis, tendinosis comes the order of the day. Therefore, these are pathologies on the same spectrum but at different levels.
On a background of tendinosis, there could be an acute inflammation and that would be termed tendinitis. Having received the cortisone injection, how are you feeling?
I hope this clarifies your doubts. I wish you well. Feel free to let me know if this clarifies your doubts.
I wish you well.

Thank you. After the shot a week ago I have less discomfort, but I still feel effects when extending my arm backwards or suddenly. If the MRI report didn't explain the extent of the tendinosis does that mean it's not significant?? I assume my continued discomfort is because the muscles or tendons are too tight I am studying stretching exercises that it seems could prevent worsening tendinosis and risks of tears.
Brief Answer:
I will recommend you continue with treatment!
Detailed Answer:
Hi and thanks so much for this follow up.
I will stick with management at this time. If full recovery is achieved, that’s great. If not, a control MRI would be able to tell us what the exact extend is and whether there is need for more aggressive measures.
Keep me updated with your progress.
