Question: 'There is comminuted fracture of the tibial spines with detached spines seen in the joint cavity.synovial effusion is seen'. Same statement is given as per CT scan report of my friend, who was met with an road accident two days ago.Whether it requires surgery or will be normal by plaster only. Please suggest all types of alternatives may be done. Also how much cost it will involve as my friend is a self made student so he is quiet low in monetary term, can you suggest any alternative through which this treatment can be reimbursed.

Brief Answer:
ARTHROSCOPY FRACTURE REDUCTION AND FIXATION

Detailed Answer:
Hi, XXXX

Thanks for your query.

I'm Dr. Sudhir Kumar,

After going through your query I concluded that your friend got comminuted fracture of the tibial spines with detached spines by an road accident two days ago. In my opinion it require arthroscopic reduction and internal fixation. Fixation can be done by Pull through suture method or cannulated cancellous (C.C) screws. I suggest that because as tibial spine has attachment of ligament particularly ACL so surgery is essential for accurate reduction and normal functioning of ligament and knee. Result of this method is excellent. Its cost varies from institute to institute with approximate average cost of treatment being nearly 50000-75000Rs in XXXXXXX Other alternative treatment are open reduction and internal fixation and closed reduction and cast application. Reimbursement of it depend on your local rules and his health insurance status.

You can discuss with your treating Doctor about it.

Hope I have answered your query.
If you have any further questions I will be happy to help.

Wishing you good health.
Take care.

Brief Answer:
Arthroscopic reduction and fixation will be bette.

Detailed Answer:
Hi,XXXX
After going through your query and follow up query I want to say that best treatment of it is arthroscopic reduction and fixation by sutures/screws. I recommend surgical treatment. You can contact in any of government medical college where cost is low. For example in our institution (GANGORI Hospital,SMS Medical College Jaipur) it's costing is between 15000-30000 Rs because there medicine are free, No doctor fee, no operation charges. Only implants are not free. It can also be treated by plaster by plastering the knee joint in extension but results are not dependable.
I can better assist you if you upload CT report. He may further need MRI of knee joint for better assessment.

You can discuss with your Doctor about it.

Hope I have answered your query.
If you have any clarification then I will be happy to help you.If you have no clarification then you can close the query and rate the answer.
