-HDL is considered to good cholesterol which helps prevent CAD and needs to be at least 40 or more in males; yours is 21 which is too low

-HbA1c level shows unsatisfactory control of your blood sugar (are you on any medications to control your blood sugar? If not, then you need to start immediately)
Based on these reports and your ECG, the conclusion would be:
-Diabetes: Requiring management, if no management has been initiated. If management has been initiated, it needs to be changed as adequate control is not being achieved
