Brief Answer:
You may require arthroscopy.

Detailed Answer:

Hi
Thanks for your query

I'm Dr Sudhir Kumar

I have gone through your query and understand your concern.
2. stellate area of adnormal signal in the posterior horn of the medial meniscus means there can be tear of posterior horn of medial meniscus .
3 partial thickness chondral defect posterior medially within the femoral condyle means there is cartilage defect in the posteromedial part femoral condyle.
4 small joint effusion means there is some fluid in the joint.
You may reguire arthroscopic assessment and if meniscus tear confirmed then meniscustomy or meniscus repair can done. I wanted to know whether you got trauma or it occurred spontaneous.
Hope your query get answered. If you have any clarification then I will be happy to help you.

Wishing you a good health.
Take care.
