Question: hello. i had a distal bicep fracture repair done. and i still have a large indent in my forearm. it hurts when i pronate and supinate. i had an mri scan and it looks like my extensor capri ulnaris humeral head and possibly my supinator is completely torn as well at the medial epicondyle. i was just wondering if you could confirm this. from my post op mri scan. thank you.

Brief Answer:
Images provided are discussed below, please send complete scan if possible

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. There are post surgery changes in area of insertion of the long and short tendons of biceps in the bicipital tuberosity of the radius.

2. The images are not too clear however there is no obvious tear noted in the region of extensor carpi ulnaris tendon attachment at the lateral epicondyle of humerus.

3. The supinator is also well visualized in its course from the lateral epicondyle and inferiorly.

4. The medial epicondyle attachment of the pronator teres is probably what you are worried about, it looks essentially normal. However in the next sections there might be partial voluming giving the appearance of an irregular contour. Behind the pronator teres we have the flexor muscles.

5. I would like to know complete clinical history and surgical details to provide you with a more accurate information that you are looking for.

A MRI scan of elbow about 100 to 200 images in DICOM format which are medical standard images to be seen at the workstation. I have the software and workstation setup at my end. Can I request you to please upload your raw images (all the files in the MRI CD provided by your hospital) to a file sharing facility like Dropbox and send me the download link. That way I can have a detailed look at all images, including images, other than the ones you have attached.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

thank you for your response. the indent in my arm was my primary concern before surgery. as i was carrying a washing machine when i lost my grip. " my hand essentially just opened" and the machine fell on my forearm causing the bicep fracture. i do not have all the mri images yet but i will send you what i have. and hopefully it will help you more for now untill i get the rest. thank you again. i have also included a picture of both my forearms so you can see the difference. thank you. Brief Answer:
There is slight injury to the skin and fat under it in the indent area

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

1. After checking the pictures of your forearms and the MRI images, there is a slight soft tissue injury in the region of the indent. However this appears to be confined to the skin and fat layers under the skin and not involving the muscles in the region.

2. Since the MRI images are acquired in the relaxed position, therefore the muscles are not is a state of contraction, which is seen in the picture.

3. The washing machine falling on the forearm is a heavy weight and you appear to be well built and therefore the impact was less violent. The primary injury was on the muscles including distal biceps tendon. It is possible that the surrounding muscles also developed mild swelling without any serious injury or complete tear. In muscle and tendon injuries, getting back to normal might take up to 6 weeks and till then you might feel weakness in the muscles.

Hope your query is answered.
