1- bilateral basal increased vascular marking more evident at para-cardiac regions, for follow up - what does it mean exactly?
2- small nodular opacity is noted at left upper lung zone possibly due to old granulomatous changes what does it mean ?

3- cardiac shadow is not enlarged in its transverse diameter
4- costophrenic angles are free

Major finding is increased bronchovascular markings in paracardiac region(area adjoining heart) in both sides.It may be due to many causes like infection/inflammation etc.
This need clinical correlation.
There is also a small nodular opacity in left upper lung zone.It may be due to old granulomatous changes.This is a benign finding and most likely it is healed stage of past infection.Generally it doesn't require any treatment.
Other findings like CP angles,heart size are normal.
In short,findings are not serious.You may need follow up X-rays for increased bronchovascular markings.

Hello
You need proper clinical evaluation.It is important to know your symptoms and reason for chest X-ray.
If there are any additional sounds in chest during auscultation like wheeze,ronchi,decreased respiratory sound etc then you may need antibiotics,bronchodilators etc ,otherwise in asymptomatic cases you may just need repeat X-ray after three months.
CT scan and PFT may be needed if symptoms persists.
Routine hemogram should also be done.
