Question: Ultrasound of abdomen says that liver is echogenic and attenuating consistent with diffuse hepatocellular disease. There is an ill-defined hypoechoic area adjacent to the middle hepatic vein, measuring 1.5 x1.2x1.5cm. No intra or extrahepatic biliary dilatation is identified. Main portal vein demonstrates Hepatopetal flow.

Here is an explanation of your ultrasound result:

1. Echogenic liver and attenuating consistent with diffuse hepatocellular disease:
This means that the liver structure seen in the ultrasound is not even. Between the healthy liver cells there are some structures (which are not a liver cells). This structures are all over the liver. Its like a mix of liver cells and other tissues put in random way. This other tissues is most likely a fat or fibrous tissue. Such image is seen in case of fatty liver or liver cirrhosis. But it is impossible to give exact reason basing only on the ultrasound result. Liver biopsy can answer what exactly is seen in the ultrasound.

2.Ill-defined hypoechoic area adjacent to the middle hepatic vein, measuring 1.5 x1.2x1.5cm:
There is some structure seen near the hepatic vein. It is ill-defined, what means that the image is not clear. So it is hard to say what exactly it is. It needs further examination.

3.No intra or extrahepatic biliary dilatation is identified.
In simple words bile ducts are regular, no dilatation, no stones, nothing is wrong with them.

4.Main portal vein demonstrates Hepatopetal flow.
This describes the direction of the blood flow in the portal vein. Hepatopetal is fine, the blood flows constantly forward. There is no portal vein hypertension.


According to this ultrasound result, most likely you have fatty liver or liver cirrhosis. As the ultrasound is very sketchy test, I suggest you to have abdominal CT done to see what is the area close to the hepatic vein. You should also consult hepatologist and consider liver biopsy.
