Brief Answer:
Suggests Renal Paranchymal disease

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that your routine blood tests have revealed rise in Urea ,creatinine and phosphate levels and fall in GFR .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you any urinary symptoms like Frequency for urination ,nocturnal frequency.poor stream ,urgency or hesitancy etc since last 6 months ?
2) Do you have swelling of the face and legs ?
3) Do you have any general symptoms like loss of appetite ,fatigue etc?
4) Are you diabetic?
5) are you taking any medication for other illness ?

The cause of altered levels of these values in blood and fall in GFR suggests chronic Renal failure the cause of which needs to be investigated .It could be either due to chronic back pressure changes in kidney secondary to Benign Enlarged Prostate or due to Chronic Renal Paranchymal disease .

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Serum Electrolytes.
5) Blood sugar Levels

If needed he will refer you to Nephrologist for opinion as regards Chronic Paranchymal Kidney Disease (CKD)..

Further treatment will depend upon result of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.
