In my opinion there is no need to worry about the fibroid and PCOD in ultrasound report.
Because these fibroids are seedlings means small size and they might not cause any problem in conceiving or in pregnancy. They might be an incidental finding in a few people.
Coming to PCOD which is a polycystic ovarian disease, here there is problem with ovulation. So clomiphene which you are taking is useful. Clomiphene causes ovulation in 80% and there by increasing the chances of pregnancy.
Also clomiphene along with HCG injection increases the success rate in PCOD. So you can discuss regarding this with your doctor.
Participate in intercourse daily if possible for better results.
Also if you are overweight or obese try to decrease the weight by doing exercise daily and avoiding junk food and including nutritious food. Because even 10% loss of weight causes the cycles regular with regular ovulation naturally.
Nothing to worry about them because due to alteration in lifestyle and stress during these days PCOD is routinely seen. With apt treatment and weight reduction conception occurs.
At last the most important thing is leave aside anxiety and stress as anxiety itself is one of the causes for infertility.

Thanks a lot for your inputs, it was really helpful but still i have few queries.Please advise on the below.
Actually my only concern is , When i took the follicular test by Oct 2014 i.e 3 month prior I had everything normal, uterus was normal in size and echotexture. Today when Follicular study is done, this polycystic configuration is shown in my report. So is this a side effect of clomid ? as food habbits are really the same from oct to till now. Just wanted to know whether in case if my periods come this month does this polycysts go away ? How to consider this .

Here are the answers for your queries:-
In my opinion the fibroids and polycystic might not be the sideeffects of Clomid. PCOD doesn't go after periods.There are different treatment options for PCOD like oral contraception pills for 4 to 6 months.If medical method is failed then laparoscopic puncturing of cysts is done.But lifestyle modification like weight reduction is first and most important in PCOD.
Regarding food habits for both avoid junk food and include healthy and nutritious food.Include fruits and veggies in diet which are rich in antioxidants.Antioxidants are helpful to body and also help in conception.
No specific food is needed during intercourse by both.
The headache and constipation might not be the sideeffects of clomiphene but they might be nonspecific symptoms and need not worry about them.
The endometrium thickness of 6.9 mm is normal.The right dominant follicle size is normal.Dominant follicle grows at the rate of 1 to 2 mm per day till ovulation.And after ovulation it shrinks and size decreases.There is no definite size for each day.But just before ovulation mostly it comes upto 20mm.
Cold and scanty bleeding wont effect the ovulation.
So relax the follicle is growing normally.
