Regarding your concern, I would explain that a similar fluctuating pattern of heart rate means that both brady-arrhythmia and tachy-arrhythmia may be present.
In such case, it is necessary to define exactly the proportions of fast and low heart rate, and also their relationship during a day-night cycle.
We have to do only with a very low sinus rhythm (bradycardia) or also a pathological atrio-ventricular conductance disorder, it remains to be stated by the Holter conclusion.
Coming to this point, if a pathological conductance disturbance is present, in conjunction to episodic tachyarrhythmia (atrial fibrillation, paroxysmal supraventricular tachycardia, etc.), then a Sick sinus syndrome may be the most likely diagnosis.
Nevertheless, additional medical tests such as:
- a cardiac ultrasound
- thyroid hormone levels
- blood electrolytes levels
- fasting blood glucose levels
- cardiac stress test
- complete blood count
- PCR, sedimentation rate
need to be investigated before coming to an etiological explanation (regarding the possible causes).
