What Does The Following Semen Analysis Report Indicate?

Posted on Mon, 8 Jun 2015
Question: I have been to see fertility specialist. She suspects I have polycystic ovaries and hence do not ovulate. I have had vaginal swabs and an internal examination today. (Everything has come back normal). Ultrasound scan is also normal. Blood results are as follows: Estrogen 115, LH 1.7, FSH 5.6 (taken on Day 2 of cycle)

Ultrasound scan shows multiple follicles and endometrial thickness of 8.4mm. Is this normal? What should the range be?

I have no reports to upload.

Husband semen analysis is also good. With a 5% morphology which specialist says has to be above 4%

My bmi is 45 and she wants it down to 30 before commencing any type of treatment which she said will probably be tablets to make me ovulate. I have currently lost 8lb.

I am worried if I don't lose weight I will never get pregnant. Clearblue fertility monitor in the past has shown ovulation and some months no ovulation do you think I could keep try to use this. This month Clearblue fertility monitor showed 3 bars on day 11 and 12 of cycle. When should I have a progrestorone blood test to make sure I ovulated?

I usually have a regular bleeding pattern 24-28 day cycles. Never have pain or any kind of problem. But my bleeding cycle this year has been very erratic and I am worried sick why. Bleeding pattern this year has been:

JANUARY 2015
23rd January - 27th January (5 days bleeding)
Cycle 24th December – 22nd January = 30 days

FEBRUARY 2015
NO PERIOD

March 2015
4th March – 8th March (5days bleeding)
Cycle 23rd January – 3rd March = 41 days

April 2015
9th April – 13th April (5 days bleeding)
Cycle 4th March – 8th April = 37 days

MAY 2015
8th May – 12th May (5 days bleeding)
Cycle 9th April – 7th May = 29 days

1. Are my bloods normal?
2. Do you agree the fertility monitor is a good diagnosis for ovulating?
3. Do you see I can conceive naturally?
4. Any suggestions please to help me achieve what I really want.
5. Am I stressing out to much?
6. Why do you see such a change in my cycle? I don’t see Fertility Specialist for another 3 months as she wants to give me time to lose weight.
7. HSG was requested but I can’t have due to anaphylaxis to Iodine, is there any other way of telling if my tubes are blocked please?

If Clearblue fertility monitor shows 3 bars do I:
1. Ovulate
2. Release an egg
3. Should I conceive naturally

Another thing that confuses me is Fertility Monitor shows Ovulation, but Ovulation strips do not. I think I am getting myself into a Frenzy and should just use the Fertility Monitor and forget ovulation test strips. What are your thoughts please? Ovulation strips only ever show 1 line. Fertility monitor is working as it should and shows 1, 2, 3 bars (low, high and peak fertility)

Brief Answer:
You will conceive naturally

Detailed Answer:
Hi XXXXXXX

Good to see you back.

You have good chances of pregnancy as long as you lose your weight.

Ultrasound scan report shows you have good number of eggs which is good thing. I am glad to see ultrasound scan report.

Use fertility monitor which is a good reliable way to test ovulation. You don't need HSG as we discussed.

Weight loss is only thing required.

Let me know if you need any more help.

Regards,
Dr. Mahesh Koregol.

Brief Answer:
Endometrial thickness is normal

Detailed Answer:
Hi,

Endometrial thickness more than 7is good enough. Infact low endometrial thickness less than 7 mm is bad and worrying.

You have 8.4 mm which is fine. Not to worry. Hopefully follicles also grow along.
