What Does The Following Semen Analysis Report Indicate?

Question: Semen analysis in the Month of October 2016

Time of collection. 9:30 am ( delayed liquefaction
Period of abstinence : 5 days
Quantity : 2 ml
Appearance : opaque
Odour: XXXXXXX
Reaction: alkaline
Fructose test: positive

Total sperm count: 25 million per ml
Motility
On collection : 30 percent
One hour after collection : 25 percent.
Total abnormal sperm: 30 percent
Pus cell: nil
Red blood cells : nil.

Thereafter I have done a Semen Analysis in February 2017

Time of collection. 8:40 am
Period of abstinence : 3 days
Quantity : 2 ml
Appearance : opaque
Odour: XXXXXXX
Reaction: alkaline
Fructose test: positive

Total sperm count: 25 million per ml
Motility
On collection : 45 percent
One hour after collection : 30 percent.
Total abnormal sperm: 30 percent
Pus cell: nil
Red blood cells : nil.

We have been trying to have a child since last one year. I have also done Semen Culture and Sensitivity on 14/Feb/ 2017. The said test showed staphylococcus. I have visited the doctor who has given antibiotic ofloxacin for 5 days from 15tth Feb night.

Kindly advise me on the said reports. There has been an improvement in the motility from 30% to 45% prior to commencing the antibiotics.

Thanks for query .

I have gone through our previous discussion on this issue in Nov when your semen report revealed delayed liquefaction suggesting to be due to infection .Accordingly I had advised you to take Doxicycline for 4 weeks .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Have you taken Doxicycline for 4 weeks ?

Your semen culture done very recently on 14 Feb shows Staphylococcus infection but there is no mention of liquefaction time ..

Semen on ejaculation is thick and sperms are unable to move freely in semen It is after liquefaction effected by an enzyme Psostatic Specific Antigen(PSA) secreted by prostate the sperms become freely XXXXXXX and move forward in to uterine cavity and then in fallopian tubes where fertilization takes place resulting into pregnancy. This explains the importance of Liquefaction time in movements of sperms in vagina after semen gets deposited during intercourse .An liquefaction time of more than 60 min can reduce the chance of conception, because the sperm cannot start travelling.

Since your recent semen culture has revealed infection due to Staphylococcal organism you need to take appropriate antibiotic as per sensitivity report at least for 6 weeks .

I would be happy to go through your semen culture report to make specific and precise suggestions .

I would suggest you to consult qualified Infertility Specialist for detail evaluation and do test called Post Coital Test which will help to determine the effect of delayed liquefaction time and need for any further treatment like Intra Uterine Insemination or IVF.

i have not taken doxicycline. i have been given ofloxacin as stated in the culture report. why do u say 6 weeks?. i have been told for 6 days. is the infection curable or is it serious issue.

my motility has increased to 45% from 30 earlier. the new report did not show delayed liquefaction as seen in the earlier report.

i have also started taking Sensa q 10.

Semen infection takes long time to get eradicated completely .Hence one has to take antibiotics for 6 weeks this protocol is scrupulously followed by almost all infertility specialist .

The infection can be eradicated completely with appropriate antibiotic reported in semen culture and sensitivity report .Hence it is mandatory to take Ofloxacin for 6 weeks .

Your recent report has revealed Staphylococcal infection .

Will the cure of the infection increase the count and motility?
