
What Does The Following Ultrasound Report Indicate?

Question: Hi, I am a 52 YO female and have been post menopausal about 2 years. I recently experienced some post menopausal bleeding so my doctor ordered an US. I received the results from my US but I'm not sure what this all means. My doctor just said that there were no specific finding on the ovaries or uterus. I was just wondering if I should be concerned about these results and should consider additional follow up. Right ovary: 2.0 x 2.1 x 1.2 cm. There is a normalfollicular pattern. Normal color doppler flow in the ovary.There is small localized echolucent ascites medial to theright ovary measuring 3.4 x 2.4 x 3.4 cm. A single thinperitoneal band courses across the fluid. The fluidoutlines a few loops of bowel.Left ovary: 2.2 x 1.2 x 1.2 cm. There is a normalfollicular pattern. Normal color doppler flow in the ovary.There is no pelvic free fluid.

Thank you for that followup - this is very helpful. So, before you get to side-tracked by the "other" findings in the pelvis, please consider the reason for the ultrasound in the first place. It was because of an episode of post-menopausal bleeding. The reason that this is of concern is that in a number of women, this is how uterine cancer comes to our attention. So, the evaluation is related to ruling out uterine cancer - try not to forget that. It is well known that in women with postmenopausal bleeding, an endometrial stripe of <4mm is NOT associated with uterine cancer. This is great news and in many cases with this finding, I would defer the biopsy of the uterine lining as it is not necessary.
With regard to the comments about the ovaries and small amounts of fluid in the pelvis, this all sounds relatively normal and does not necessarily warrant followup (and consider that these findings are not related to the original problem, they are incidental findings). At the very most, I might offer a patient a followup scan in 3-4 months to make sure that the fluid in the pelvis is stable / gone.
So here is what I would recommend:
1. You should be very reassured by these findings - you do not have uterine cancer
2. The most common cause of postmenopausal bleeding is thinning of the vaginal walls and thinning of the uterine lining - they are all prone to small amounts of bleeding
3. If you have ANY additional bleeding, then you need to ask for the uterine biopsy
