What Does The Following Ultrasound Report Indicate?

Question: my daughter ultrasound says the uterus is deeply arcuate in shape and avascular thickening at the fundus 9.6mm in the supero-inferior dimensions. The uterus is anteverted and measures 81x3643 mm. the mymetrium is normal. The endometrium is 14 mm thick. No gastational sac is seen.
The right ovary shows a corpus luteum 11 mm across.
The left ovary is not visualized(she had ealier ovary cyst that was removed with surgery)
There is a solid mass in the right adnexa measuring measuring 11x8 mm across. It shows a mild vascularity and 02mm central cystic area. No embryo is seen. findings are suggestive of ecotopic pregnancy. there is no free fluid in the pelvis. Dr. has suggested to wait and watch for 48 hours.
My worry is there any life risk in getting her pregnant . her uterus is also arcuate. and chances are she may face again with ectopic pregnancy.
should she avoid for future pregnancy . she is 24 years

I understand your concerns for your daughter.

Considering her current pregnancy to be ectopic, immediate medical or surgical management is required as there is always a risk of rupture.
There is no point in waiting for 48 hours if ectopic pregnancy is confirmed.

Arcuate uterus is not a risk factor for ectopic pregnancy in furture.
But there are chances of recurrence of ectopic pregnancy as she is currently having an ectopic pregnancy. As many as one in 10 women will have a second ectopic pregnancy.

She can go for conception in future but careful monitoring is must.

If she miss a period and might be pregnant, let her doctor know immediately.

The first step would be to confirm if she is pregnant and where the pregnancy is located.
This may be difficult early in the pregnancy, so she should be aware of the early signs of an ectopic pregnancy, such as pain and vaginal bleeding or spotting.

Dear Dr. XXXXXXX

Thank you very much for guiding us.. The decision by the Dr. was taken based on the size of the spot after doing the test B HCG. She has advised, similar test will be conducted tomorrow and if needed, this will be dissolved with medicine, otherwise it should automatically dissolve. Please advise.

Also my worry is, should she plan for pregnancy for future as the risk factor is 10% for ecotopic preg.

Also I mentioned she had undergone surgery for benign ovary cyst in 2014 where her left ovary was removed.
Will she be able to conceive normal in the uterus with one ovary (does chances become more for ecotopic preg) in this case. May be her fallopian tube is damaged which we don't know. What test should be conducted to know this. Please guide us properly. We don't want to put her life in danger.

Will be really grateful for your detailed advise.

Sorry for the late response.

I advise an immediate action for ectopic pregnancy. beta-HCG level testing after 48hrs will help only in ruling out intrauterine pregnancy. As size of sac is small it can be treated medically.

There is risk for ectopic pregnancy in future as well but still, she can plan for future pregnancy. But the single ovary is not a risk factor for ectopic.
Also, after the medical treatment of ectopic now, she should avoid conceiving for atleast next 3-6 months.

There are chances that she can face difficulty in conceiving because her left side ovary was removed and in right side she has ectopic pregnancy which can disrupt right tube anatomy.

Hysterosalpingography (HSG) will help to detect the patency of tubes. It is an outpatient test. She can go for it if there is difficulty in conceiving in future.

Dear Dr. XXXXXXX

Thank you very much for guiding me. Just to apprise you we visited the
Doctor today and us after seeing the report of Beta HCG today which is reduced from 50.2 to 11.2, has mentioned there is no medication required and it will automatically disolve. She has now called us after one week with the another report of HCG. and also will advise us for future pregnancy.
