What Does The Following Urine Analysis Report Indicate?

Question: Hello! I want to ask something about a problem that my 3 year old daughter had some days ago. She had an improvise urine infection after a virology infection. I have made the analysis and than uriculture, which I get a response only today. Meanwhile I began giving her antibiotic Amoxilin, 7 ml three times per day. Today is the 5 day and she is feeling better, she doesnt have pain no more and no other symptoms. My pediatrian told me to continue for 7 days but this is kind of antibiotics can be given 5 days or 10 days. Now after taking the uriculture exam I have 2 questions. 1 Does this antibiotic make effect? 2.Should I continue giving her until 7 days? Thank you. She had 100 leucocites instead of 2 for field

I have gone carefully through your query and understand your concerns. The pus cell count in urine is important. I would need to know how much it was. I would also need to know the leucocyte count in blood and the differentials for the same (how much neutrophils). [100 leucocytes where? What is 2?]

The antibiogram incidentally has not tested for amoxicillin which you have been giving. So, it is hard to comment if it is effective. It is not that antibiotics like this cannot be given for 7 days and should be given for only either 5 or 10 days. I would not only advise you to continue it for 7 days but also to add another antibiotic as per the sensitivity report. Cotrimoxazole or cefuroxime are sensitive and are good alternatives. Ideally the combination should be given for 14 days. The duration may depend on the pus cell count.

You also need to give her plenty of water to drink.

Thank you for your answer. I start giving this kind of antibiotic because my girls pediatrian adviced me to and I made the uriculture by myself cause she didn't tell me to do. Meanwhile I am sending the other exam that I made which show the number of leucocites. It is not that are too much antibiotic?It is the first tume that I give to her. I think that they have done aome effect since she does not tell me that has pain during urinating. What if I continue the cure for the other 2 days and than make another analysis before starting again any other antibiotic?:

Thanks for writing back. The urine report shows a pus cell count of 2-3/hpf which is normal and does not call for treatment with antibiotics unless the colony count of E.coli is more than 10^5. Unless that is the case, you should stop antibiotics altogether after 5 days (from beginning) and need not add the second antibiotic.

Antibiotic does not count as 'too much'. It is either 'needed' or 'not needed'. It should be given in proper dose.

Just give her plenty of water to drink. She will be fine. You need not repeat the urine test unless you are too apprehensive (as the first one was normal anyway).

So with the analysis that you are seeing you are telling me to stop giving antibiotic as she is fine? Where should I see the colony count of E.coli, is that another exam? Should I stop or continue till the 7 day?

Please check the colony count of the bacterial growth. It is a part of the same test. If the report indicates that the number of E.coli is less than 10^5/hpf, you can stop antibiotics. If not, it should be given in combination to the other antibiotic.

Yes, i've seen it.

Thanks, this is what I was asking for. The colony count is more than 10^5 (i.e. 1,00,000) and so Amoxycillin as well as one among Cotrimoxazole or Cefuroxime needs to be given for a total of 14 days (under supervision of her pediatrician).
