What Does The Lab Test Report Indicate?

Posted on Thu, 7 Jan 2016
Question: Hello doctor,
My life comes around the same route again and again sadly. Please refer my previous question. Here is the answer from the doctor,

"I want to assure you further regarding HSV-1 that the virus dies and never becomes active at ROOM TEMPERATURE and IF IN AEROSOL like in sneezing or coughing even if there is an active infection in the person who sneezed."

Let me explain my current situation now. I recently moved to a new city and met a friend a month ago. We hang out a lot as we have similar interests. My friend always sparkling the saliva while he talk or he laugh loudly. There were five or six times he sparkled his saliva on my lips area as we eat lunch together and we sit next to each other during the lunch time. He could have sparkled it more times too as i never pay attention to it as he is my friend. Today he was sad and i inquire about it. He showed me his medical report that he is diagnosed HSV 1. Here is the reading from his report.

HSV 1 IGG TYPE SPECIFIC AB >5.00 HIGH

Value Interpretation
----- -------------
<0.90 Negative
0.90-1.10 Equivocal
>1.10 Positive

It seems that he has high level of herpes virus on his body. Please let me know what should i do in this situation. Is ">5" means he is very active on the virus? I am certainly scared in this situation. Please guide me as early as possible. There are two things i want to add it here. 1. I started to hang out with him more only last two weeks. 2. He is certainly not normal while he talk than other people as he sparkle saliva very badly.

Brief Answer:
IgG titre is not directly proportionate to the severity of the infection.

Detailed Answer:
Hi,
Welcome once again to HCM.

Once again I assure you that HSV 1 infection is not a life threatening one and a passing cloud in one's life.
Splashing of saliva during speaking to someone happens to everybody. Eventhough he is HSV1 positive his saliva need not contain the virus always. Viral shedding is possible only for few days prior, after and during any episode and not ever. Even if there is viral shedding it is not neccessary to be infectious to all at everytime. All the individuals exposed to a virus are not getting the infection.
Regarding the test report it indicates that your friend had encountered with
HSV 1 infection. It is not a recent episode as IgG is positive and not IgM. Moreover, the presence of IgG does not indicate that the disease is active. Just indicates that he had the infection at onetime.
The titre of >5 is not directly proportionate to the severity. Certain individuals would react to any antigen or foreign substance vehemantly whereas Certain other's body would not react so much.
So you need not worry about these values.
As long as you are symptom free you are safe.

Hello doctor,
Thank you. My friend told me that he never had any blisters on his mouth. Is it normal for people to have HSV 1 without symptoms? Do i need to test after a month?

Brief Answer:
You don't need any further test

Detailed Answer:
Hi,
Welcome back
Nearly 40% of any population have antibodies against HSV 1 infection. Among which most of them have no active lesions. It indicates that the individual has come across the infection. That's all. Nothing to worry. You don't need any further test.
