Question: Hello doctor
Good day,
My father is aged 61
He is having suffering of chronic liver disease and pht
His albumin level is 2.0
Total billrubin 10.5
Direct billrubin 9.7
Indirect 0.8
Sgot 297
Sgpt 225
Please let me know what should I do to decrease billrubin level and lncrease albumin level
Please advise

Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:

1. He is suffering from chronic liver disease. So, further any hepatotoxic drugs needs to be avoided. Until now, what ever the cause of present liver disease needs to be completely avoided.
2. Protein is low. Low calories with protein in the range of 1-1.5g/ kg body weight is necessary. If further increased, risk of ammonia toxicity present. Due to fat malabsorption, secondary to redued bile, he might have episodes of diarrhea.
3. If albumin further falls, 20% albumin infusion maybe necessary. Try to correct with appropriate protein diet and avoid excessive intake.
4. As liver function improves, jaundice should reduce. Do follow up with serum bilirubin level.

At present, calorie restricted, moderate protein diet is necessary. Adequate water intake along with moderate fibers in diet. Avoid excessive fatty food.

Hepatoprotective agents like Liv-52 or other Ayurveda medications could be helpful.

If bilirubin is further increasing, plasmapheresis may be necessary.

If it further deteriorates, then liver transplantation maybe necessary.
