I have read your query ,seen your report and understand your concern.
After reading your clinical history & report it appears that you are most probably having chronic sinusitis with neuralgia . In my opinion , your pain is trigger by nose infection .
In 2009 CT scan report , you are having chronic sinusitis with associated allergy & nasal septum deviated to left .
In 2015 CT scan report , you are having right sided sinusitis with sphenoid sinus involvement with nasal septum deviated to left . Your frontal sinuses are not developed fully .
If I were your treating Doctor for this case , I would advice you
Saline nasal spray or rinse thrice daily ,
Tab Paracetamol for pain with headache &
Antihistaminic Fexofenadine like tab Allegra at night .
You will get relief .
If relief is not upto satisfaction level then you will need antibiotics & Combination Steroid nasal spray with antihistamine 2 puff each nostril twice daily. For which you will need prescription.

Thanks for the follow up query.
In 2009 also you are having infection with associated allergy . But now it has involved sphenoid sinus which causes more headache .
I would advice use little more diluted Saline rinse with Steam Inhalation twice daily & tab Allegra at night.
Yes , antibiotics with anti-inflammatory & saline rinse will decrease mucosal thickening .
No need to remove cyst or polyp since it is not blocking sinus opening fully .
Neuralgia is a pain along the nerve distribution due to stimulation . In your case nose infection might stimulating nerve in nose . It may be associated with migraines .
You will need anti inflammatory analgesic like Paracetamol + Ibuprofen combination .
