Question: Hi

My partner recently had a sperm test done. Result where has follow


TEST - MAY 14

Volume - 3 ml
Count - 18m/ml
Mortality - 68%
Morphology - 2% (kruger)
Progression - 2
Anti sperm antibodies - negative
Agglutination - negative.


TEST 2 - OCTOBER 14

Volume -2 ml
Count - 10 m/ml
Mortality - 72%
Morphology - 4% (kruger)
Progression 2-3
Anti sperm antibodies - negative
Agglutination - 10%


I am 36.5yrs of age and healthy, with a regular cycle. All results came back consistent with age.

What is the prognosis of a natural pregnacy with these result.

I read your query and I understand your concerns.

Following is my reply:

1) The count , motility and morphology are on lower side.

2) It is difficult to conceive naturally with above semen analysis report.

3) In view of above report and your age, I suggest you undergo procedure called ICSI ( Intracytoplasmic Sperm Injection) where your eggs will be fertilized outside the body in laboratory.

I was under the impression that his Mortality result where the positive aspect. The problem was the morphology and count.


How successfully is icis, with our results.

Also what are your thought on the 10% aggulation. Is this something to worry about

Should my partner continue to take proxceed. Plus ?

Any thoughts on why his count has dropped from 18m to 10m.

And what can he do to improve the count.

Hi,

In first attempt itself we will come to know the oocyte quality and can assess about success and whether second cycle is needed with your eggs.

He can continue with medicines.

The drop or increase in count by 5 to 10 million is normal and no reasons attributed. But overall the range itself is lower count side.

Hence ICSI is best way forward.

Age - 36.5

AMH -   16.7 pmol/l
FSH – Not done
3D Transvaginal ultra sound

Right Ovary
AFC – 5 (plus Corpus Luteum 22 x 18).
Size – 39mm  x 21 mm x 34mm
Volume – 14.6cm3


 Right Ovary
AFC – 8
Size – 25mm 16 mm x 24mm
Volume – 5 cm3

Pelvic scan day 18 of cycle. Uterus XXXXXXX with good uterine artery blood flow. A fundal posterial fibroid was seen measuring 18 x 11 x 9 which is not affecting the cavity.

Endometrium = 6.5mm
