What Does The Survival Rate In A Prostate Cancer Patient Depend On?
Question: My prostate was surgically removed eight years ago (January 2008), showing malignant cells at the margins. Since that time I have had radiation to the prostate bed (spring 2011) and then radiation to deal with pelvic lymph nodes (November 2013) My most recent PET scan (September 2016) indicated that my prostate cancer has reached pelvic, abdominal, and chest lymph nodes as well as bone areas in the shoulder, rib, and thoracic spine area (although previous scans indicated that the spots on my bones were indicative of previous injuries and not cancerous) My oncologist is recommending that I combine chemotherapy (3 week cycle) with Lupron, citing recent studies that indicated that the combination has yielded better results than doing the androgen deprivation alone and then the chemotherapy. I am 66 years old and I am quite physically active (primary pitcher in fast pitch softball league 3 x week, working out regularly at the gym, walking, playing as catcher in a summer baseball league, as well as playing and coaching softball leagues and a high school boys basketball team) including an active sex life (assisted in part through the use of "the pump" and Cialis) Last spring I underwent a catheterization procedure in which a stent was placed to repair a 95% blockage of the LAD. In other words, I am currently enjoying robust physical health and I am truly concerned that I will throwing away my good health in exchange for becoming sick and exhausted and unable to do any of the things that bring XXXXXXX and meaning to my life for at least the 24 weeks of the treatment and possibly indefinitely. I am trying to gather as much information as I can right now, understanding that each doctor has biases in accordance with their respective specialties. What would you recommend?

As of now you have a widespread disease and as per the evidence , you deserve a combined therapy with androgen blockade as well as chemotherapy. This evidence comes from the chaarted and stampede trials of ca prostate and it has shown that combined treatment offers survival benefit. So u can trust your doctor and go ahead wuth combined treatment. Chemo will be given for 6 cycles and then androgen blockade will continue.
