What Does The Ultrasound Sound Report Indicate?
Question: I had ultrasound for heavy bleeding and spotting. Here are results:
The uterus is mildly enlarged, measuring 8.8 cm in length, 4.7 cm AP
and 6.3 cm transverse. The myometrium is heterogeneous and mildly
hypervascular, question adenomyosis and/or fibroids. There is a
single defined fibroid or adenomyoma in the anterior left myometrium,
3.4 cm in size. The cervix contains nabothian cysts but is otherwise
unremarkable.
The endometrium is homogeneous and regular, 6 mm thick.
What exactly does this mean? Is hypervascularity something to worry about?

This hypervascularity is because of fibroid and adenomyosis present.
I would like to know your age and also if you have completed your family.
Also I would like to know if there is change in cycle length recently.

I have had four pregnancies/four (big) children. I am 42. My cycle length is probably five days but I have one or two days of very heavy bleeding plus I'm spitting during other days. Any thoughts on this?

Considering the presence of fibroids and adenomyosis along with symptoms of excessive bleeding and even intermenstrual bleeding with complete family, I will recommend you to opt for hysterectomy as soon as possible.
Most probably all your symptoms are due to fibroids only but nevertheless chances of cancerous growth cannot be ruled out till a histopathological examination is done.
In case you are reluctant in opting for hysterectomy then at least get an endometrial biopsy done so that endometrium can be evaluated by doing histopathological analysis of the biopsy.
