What Does This CT Scan Report Indicate?

Question: My father is 78 years old. Few years ago he had prostrate cancer and had an operation. His PSA is now about 8. And CT scan showed heterogeneous signal intensity of the prostrate gland. The gland is 20 X 20 X 25 mms. No evidence of pelvic lymphadenopathy or metastatic bony lesion. Has the cancer recurred again?

Thanks for your query,based on the facts that you have posted it appears that your 78 years old father who has undergone surgery of Radical Protatectomy few years back and is reported to have PSA of 8 ng and CT Scan has revealed heterogeneous high signal intensity of the prostate.

The high intensity signals in prostate and marginally raised PSA (8 ng) strongly suggest Chronic Prostatitis .However the possibility of recurrence of cancer prostate has to be ruled out by doing Multiparametric MRI scan .or Contrast enhanced MRI .

The results i shared were of an MRI and not CT scan. Do you think cancer has recurred again?

Thanks for follow up .
My comments were based on your initial statement in which you have mentioned C.T Scan and not MRI .

Cancer of the prostate is usually marked as low density signals (Hypo density )in transitional zone where as Heterogeneous signal intensity signifies Chronic Prostatitis .

In view of absence of involvement of pelvic lymph nodes or distant metastasis and long period after surgery it is unlikely to be recurrence of cancer .

You need to do Serum P.S A estimation at frequent intervals of every month to be rest assured about recurrence .
