Brief Answer:
Appatently he does not need surgery

Detailed Answer:
Hello,
I am Dr.Rajesh Teli,MD.
I have gone through your query. Thanks for using HCM.
Your father has got hypertension and Diabetes mellitus which appears under good control. His echocardiogram shows age related thickening of valve with minor leak. This is not significant and needs no surgery .His concentric LVH with diastolic dysfunction may also be related to ageing.Though it may be effect of uncontrolled hypertension .so he should regularly monitor BP and it should be maintained below 140/90.
His blood reports are showing low sodium which should be corrected in consultation with your treating doctor.
Having done that he should get exercise treadmill test done to rule out blockage in arteries of heart as a cause of shortness of breath. If TMT is normal, he only needs control of BP and sugar .
My best wishes
Dr.Teli.
