Your are having Rotator cuff tendinitis due to Impingement syndrome and arthropathy of AC joint. This could indicate that there may be sharp bony points under the acromion which is scratching against the Supraspinatus tendon and causing inflammation or tendinitis.
I would analyse the space between the Acromion and the Supraspinatus tendon. If it is less then I would recommend an arthroscopic or key hole surgery sub acromial decompression to increase this space so that the tendon does not get eroded further and then physiotherapy will help bring back the strength of this muscle with appropriate medication and precautions.
This needs to be discussed with your Sports Medicine specialist or Shoulder surgeon to decide if physiotherapy alone is enough or combined surgery and physiotherapy is needed.
Avoid lifting weights or lifting the arm under resistance much overhead.
Apply hot packs and perform shoulder range of movement exercises.

I discussed with my.physiotherapist and orthopedic dr.and showed them my mri reports.they told me take tendocare tablet and apply heat pack and do light exercise.can u please tell how much time does this take to recover?should i show you my mri reports?
