What Does This MRI Report Indicate?

Question: I had an MRI and don't understand the results: Multiplanar multisequence MR imaging of the brain without and with intravenous contrast with 17 cc of Magnevist. No prior exam available for comparison. There is a 2 mm focus of magnetic susceptibility artifact on gradient-echo imaging in the anterior aspect of the left frontal lobe subcortical white matter near the vertex. This is dark in signal intensity on all other sequences and does not demonstrate any enhancement. Findings raise a possibility of a small amount of calcification although old blood products cannot be excluded. There is a 2 mm T2-weighted hyperintensity in the subcortical white matter of the right frontal lobe, a 3 mm T2-weighted hyperintensity in the subcortical white matter of the posterior left frontal lobe and an adjacent 2 mm hyperintensity. A 4 mm hyperintensity is present in the subcortical white matter of the left parietal lobe. None of these T2 bright lesions are oriented perpendicular to the lateral ventricles. The corpus callosum is normal. The proximal cervical cord appears normal. No cerebral edema, mass, mass effect, or extra-axial fluid collection is present. Gray-white matter differentiation is normal. The ventricles and sulci are normal in configuration. I see no evidence of abnormal enhancement or restricted diffusion. A retention cyst is present in the right maxillary sinus. The usual vascular flow-voids are identified.

I have gone through your question and understand your concerns.
The Mri report is showing multiple white matter brain abnormalities.
At this age it may be due to some primary demyelinating disease like multiple sclerosis or other causes like vitamin B12 deficiency, vasculitis, hypertension or diabetes related.
The distribution of lesion is not typical of multiple sclerosis and so other causes have to be evaluated.
What is your clinical presentation ?
Any history of visual involvement or motor sensory deficit?
Any migraine history?
Any other medical illness?
Other finding on MRI (A retention cyst is present in the right maxillary sinus) needs to be evaluation by ENT specialist.

Could this be caused by trauma? I had a bad motorcycle accident and car accident prior to this, both which rendered me unconscious for a period of time. I have also had migraine headaches in the past which is what prompted the MRI.

Hello XXXXXXX XXXX,
Yes, trauma and migraine both lead to white matter hyper intense lesions.
The MRI is also suggestive of a frontal calcification probably secondary to traumatic haemorrhage.
Most of the lesions are located in frontal lobe and may be due to concussion of trauma if the head injury is not very old.
You can also upload MRI images in pdf format for evaluation.
If no demyelinating episodes of vision loss, motor weakness, sensory loss or seizures then these findings are secondary to trauma and migraine.
You need to control migraine attacks and get some investigations like Vitamin B12 , blood sugar, creatinine and thyroid profile.
